    Description

    JOB SUMMARY:

    Assists in providing the fundamental aspects of programs or cases and assists in providing client services (childcare/transportation), including support for families, under close and regular supervision.

    ESSENTIAL FUNCTIONS:

  • Childcare for the clients attending treatment services
  • Assists with transportation needs as necessary
  • Assists with conducting programs to prevent and resolve client problems.
  • Provides information to referred clients over the phone or in person
  • Provides case management assistance, . preparation/maintenance/scanning of client files, documentation in Millennium, mailings such as outreach, client tracking of services/record keeping in the EHR, and technical and administrative assistance.
  • Assists clients in identifying/linking to benefits, social and community services and helps clients obtain the resources.
  • Provides support to clients.
  • Assists with cleaning the childcare room and preparation for the next day

    KNOWLEDGE/SKILLS/ABILITIES:

  • Basic knowledge and understanding of case management
  • Basic computer and software skills and experience
  • Complete work with accuracy and meet deadlines
  • Communicate effectively, both verbally and in writing; understand and follow directions
  • Provide a high level of customer service
  • Establish and maintain effective working relationships with clients, families, caregivers, service providers and staff
  • Knowledge of community resources is highly desirable

    Position Requirements

    MINIMUM QUALIFICATIONS:

  • High School diploma;
  • 1 year of related experience (work or volunteer).

    • LICENSES/CERTIFICATIONS NEEDED: Valid driver's license (if applicable)

    MANDATED PROGRAM REQUIREMENTS:

  • Medical clearance, drug screen and fingerprinting required
    • Exempt/Non-Exempt Non-Exempt Weekly Work Hours 36 Proposed Salary $36,000 to $40,000/annually ScheduleMonday - Thursday 8:30 AM to 4:30 PM and Friday 9:00 AM to 1:00 PM
