    Overview of the Role:
    Seeking a passionate Nurse Practitioner to join our care team in Florida. This position plays a crucial role in delivering comprehensive healthcare services to patients throughout their care journey, enabling a positive impact on patient outcomes and care continuity.

    Key Responsibilities:
    • In-Home Assessments (IHA): Conduct comprehensive in-home assessments for patients, including medical history reviews, physical examinations, and medication management.
    • Patient Assessment: Conduct thorough assessments of patients' health status, including physical examinations, medical histories, and risk assessments.
    • Diagnosis and Treatment: Formulate accurate diagnoses and develop individualized treatment plans based on evidence-based practices and guidelines.
    • Chronic Disease Management: Manage and monitor chronic health conditions, ensuring optimal disease control and patient education.
    • Medication Management: Prescribe medications, adjust dosages as necessary, and educate patients on proper administration and potential side effects.
    • Preventive Care: Promote preventive healthcare measures to enhance patient health and prevent future illnesses.
    • Patient Education: Provide guidance on disease management, healthy lifestyles, and self-care practices to empower patients in their health journey.
    • Collaborative Care: Work closely with healthcare professionals to ensure coordinated care for patients.
    • Documentation: Maintain accurate electronic health records to track patient progress and treatment plans.
    • Bedside Manner: Build trust and rapport with patients and families through compassionate care and empathy.
    • Follow-Up: Schedule regular follow-up appointments to assess treatment effectiveness and adjust as needed.
    • Quality Assurance: Participate in quality improvement initiatives to uphold care standards.
    • After-Hours On-Call: Be available for after-hours on-call care and visits as required.
    • Travel: Drive to client and patient locations within the designated area.
    • Flexibility: Perform other assigned duties based on business needs.

    Qualifications:
    • Active, unrestricted Nurse Practitioner license in Florida.
    • Certification as a Family or Adult-Gerontology Nurse Practitioner.
    • Experience as a primary or longitudinal care provider preferred.
    • Valid driver's license with a clean record.
    • Current Basic Life Support Certification.
    • Active DEA license.
    • Strong clinical assessment and communication skills.
    • Proficiency in electronic health record systems.

    Preferred Qualifications:
    • Experience with HEDIS/STAR measures, home-based visits, and HCC documentation.

    General Physical Requirements:
    Ability to perform physical tasks including navigating various dwelling conditions and conducting assessments.

    Schedule:
    Working Hours:
    Monday to Friday, 8am-5pm with a one-hour lunch break.

    Weekly Visits:
    Minimum of 25 visits per week.
