Position Summary:


A wide range of efforts are underway to improve the care of Philadelphia citizens who have been found Incompetent to Stand Trial (IST).

To meet the unique needs of these IST individuals who require judicial supervision, mental health care, and substance use services, the Re-Entry Team works to assist individuals in their transition between jail and the state hospital or a less restrictive setting, while under the court's jurisdiction.


The Re-Entry Team Certified Peer Specialist provides supportive services to individuals at PDP, NSH, WeSH or other community settings with mental illness and assists these individuals through a variety of services and roles.

This position requires a trained peer specialist with histories of mental illness and criminal justice involvement to help those with similar histories.

This forensic position requires special attention to the needs of justice-involved people with mental illness, including an understanding of the impact of the involvement in the criminal justice system on behavior.

An understanding of trauma and post-traumatic stress disorder, which are prevalent in this population, is helpful.

In addition, the Certified Peer Specialist will serve as a community guide, coach, and advocate to link newly discharged people with housing, vocational, educational, and other community resources/services.


Duties and Responsibilities:


  • Primary duties may include but are not limited to: _
  • Meeting with and engaging individuals at PDP, NSH, WeSH and community settings
  • Connecting individuals to services upon release or discharge
  • Providing case management to individuals transitioning from PDP, NSH or WeSH
  • Managing list of community resources
  • Contacting Adult Probation & Parole if needed
  • Meeting with families of individuals transitioning from jail or state hospitals\
  • Conducting outreach calls
  • Supporting and enhancing individuals' emotional stability
  • Monitoring and reviewing counseling/treatment plans
  • Tracking and collecting outcome data monitoring individuals' progress in treatment and community reintegration, and recording that data in appropriate systems

Skills Required:


  • A Certified Peer Specialist must be able to:_
  • Employ skills and effective problemsolving strategies, and respond in a timely fashion to prevent or curtail relapses and other crises
  • Provide information on the rights and responsibilities of discharged offenders and on satisfying criminal justice system requirements and conditions (probation, parole, etc.)
  • Share their experiences as returning offenders and modeling the ways they advanced in recovery
  • Support engagement in mental health and substance abuse treatment services in the community, including the use of psychiatric medications and attending 12step and other abstinencebased mutual support groups
  • Share their experiences and provide advice and coaching in relation to selfsufficiency
  • Provide practical support by accompanying individuals to meetings or treatment appointments and referring him or her to potential employers and landlords
  • Work effectively as an essential member of the Re-Entry Team.
  • Effectively use computers, databases, and related common office software packages such as Microsoft Word, Excel and Power Point

Education and Experience:


  • High School Diploma or GED
  • B.A. or B.S. degree in social science field preferred
  • Prior forensic involvement
  • Must be a former or current recipient of behavioral health services for two years or more and have achieved at least one year in recovery
  • Completed or be able to complete DBHIDSsponsored CPS Training Program

Physical Demands:


  • Job activities require mobility and stamina involving a significant amount of walking, standing, climbing stairs and sitting.

Essential Functions:


  • Build and maintain relationships with key staff
  • Meet with individuals with mental illness on a weekly basis
  • Link individuals released from jail to services
  • Maintain list of community resources
  • Monitor and review counseling/treatment plans
  • Explain plan of care to individuals and families
  • Identify gaps in reentry resources offered and work to bridge those gaps
  • Maintain accurate, confidential records documenting services provided and unmet needs using appropriate databases
  • Remain current with resource and referral information
