Job Summary
The Community Health Supervisor supports community health activities and oversees a team of community health workers.

This position is accountable for the smooth running of daily operations, achieving targeted outcomes, and client satisfaction while maintaining compliance.

This position works to ensure that the MyCare team is delivering effective person-centered care plans for positive health outcomes driven by one-to-one home-based interventions addressing social determinants of health and increasing self-efficacy and self-sufficiency.


Job Functions

Essential Functions

  • Perform as a representative of Home Assist Health upholding the company mission, vision, and purpose.
  • Works with Executive Director to project manage the implementation and execution of new community health initiatives.
  • Monitor CHW caseloads, analyze personcentered care plans, reports, enrollment timelines (duration of care), progress towards goals, and support of clients.
  • Identify and recommend opportunities to further enhance personcentered care plans for positive outcomes, goal achievement, and when appropriate readiness for graduation (completion of the program).
  • Organize and implement CHW onboarding/training and provide ongoing support and coaching to CHWs so they do their best work for clients.
  • Lead for CHW externships and apprenticeships.
  • Facilitate team meetings, keep, and distribute minutes as appropriate.
  • Prepare for and perform as team lead in case conferences (staffing meetings) with clinical partners.
  • Manage electronic and paper filing systems and documentation.
  • Create and maintain protocols, workflows, and job aids that support internal and external activities for the team.
  • Liaise with cross continuum care partners: healthcare, social services, and community partners.
  • Provide patient navigation through medical and nonmedical needs, insurance, and other resources, and when needed, advocates.
  • Serves as MyCare's enrollment specialist serving as the first point of contact for a referral partners and case assigns CHWs.
  • Review schedules in electronic health record system for timely billing and payroll processing
  • Manage team within payroll/human resource system (i.e. review time off requests, approve timecards, etc.)
  • As needed perform as back up to a Community Health Worker
  • Assist with other duties as assigned.

Qualifications

Education

  • High School Diploma or equivalent
  • Associate or bachelor's degree preferred

Skills

  • Strong communication skills
  • Naturally personcentered
  • Experience in goal setting and achievement
  • Experience in health coaching, skill building and navigation
  • Experience working with behavioral health dynamics
  • Understanding of medical terminology
  • Understanding of social determinants of health
  • Proficient in Microsoft office, and Electronic Health Record systems
  • Familiar with Medicaid services and programs
  • Requires the ability to read, write and speak effectively in English. Bilingual preferred

Abilities

  • Must be able to work independently, as well as in a team environment.
  • Must have proven ability to lead and coach/mentor others.
  • Bilingual in Spanish is preferred but not required.

Professional Certification

  • Current and Valid CHW Certification
  • Current CPR and First Aid Certification will train and certify if needed
  • Current Direct Care Worker Certified Level 1 and Level 2 (ADP and DD modules) will train and certify if needed

Experience

  • Experience and/or knowledge with special projects, coordination, and implementation.
  • Experience and/or knowledge with infrastructure and process creation.
  • Experience and/or knowledge of Home Assist Health service industry: nonmedical home care, habilitation, intervention and or clinical settings.

Background Requirements

  • DPS Level 1 Fingerprint Clearance (or be able to obtain one within 90 days hire).
  • Must pass Central Registry Check.
  • APS Registry Check at new hire and annually.
  • Notarized Criminal History Affidavit at new hire and every year.

Immunization Requirements

  • Must provide a negative tuberculosis (TB) test result on an annual basis.
  • Must comply with vaccine requirements as required by state and/or federal mandates and/or customer contract which includes proof of COVID19 vaccination.

Other

  • Requires reliable transportation and a current driver's license.
  • Must always maintain current automobile registration and automobile insurance.

Classification

  • This position is classified as Exempt as outlined by the Fair Labor Standards Act (FLSA).

Working Conditions

  • Able to lift 25 lbs.
  • Professionally work with persons of all cultures and backgrounds in close proximity
  • May require sitting or standing for long periods of time.
