_ What you will do:

_

As a Community Health Worker (CHW), you will work as a liaison to ensure appropriate care and services are provided to patients.

The position is responsible for enhancing communication between the health care team and the patient through practice based and home visits.

With the purpose of promoting positive behavior, adherence to the care plan and identifying non-medical needs of the patient.

DVCH is committed to providing a safe and healthy work environment for our employees, patients, and visitors. In effort to achieve this goal, DVCH requires all employees, regardless of position, to be vaccinated for COVID-19.

If an employee is unable receive the vaccine for medical or religious reasons, a vaccine exemption form must be completed and submitted to the Employee Health Department.

Medical exemptions will be reviewed and approved in coordination with the Chief Medical Officer and religious exemptions will be reviewed and approved in coordination with the Chief Human Resources Officer.

Requirements:

A high school diploma or equivalent is required
Proof of completion of the 120+ hour Community Health Worker Training Program or willingness to start as an Associate and complete training program
Must be able to travel through service areas
Bilingual (English/Spanish) required
Excellent interpersonal and oral and written communications skills are required
Must be able to present a positive and enthusiastic demeanor while working directly with patients
