JOB SUMMARY:
Responsible for calling and answering patient call and scheduling


appointments with their primary and specialty care providers for specific reasons and doing so in a caring and professional manner.

Clinic Lead


JOB DUTIES:
Responsible for calling and answering patient calls and scheduling appointments with their primary and specialty care providers for specific reasons and doing so in a caring and professional manner:

  • Answers phones, screen and triage incoming calls, and direct calls appropriately.
  • Responsible for scheduling appointments for patients with their primary care providers for:
  • IHA (Initial Health Assessments) for Medicare and Medicare-Medi-Cal patients within120 days of assignment;
  • Established patients with chronic conditions in need of a follow up visit, test or treatment that has not been performed;
  • Patients who have been assigned to CCCHC by a managed care plan but have not been seen by CCCHC within the year;
  • Patients who have been discharged from the hospital after an inpatient stay within 7 days after discharge;
  • Patients who have been in the ER within 3 days from the discharge from the ER; and
  • Patients who have missed medically necessary visits or referrals.
  • Schedule appointments for xrays, lab visits, vaccinations, etc. as requested.
  • Schedules/Reschedules appointments for multiple services across central city community health Center base on protocols.
  • Contacts patients to offer follow up for any missed appointments.
  • Registers new patient on eCW appropriately.
  • Calls patients to remind of appointment the day before.
  • Update patient information and patient registration information.
  • Assist with the referral process.
  • Attend all required meetings and trainings.
  • Consistently demonstrate and uphold CCCHC's principle of providing quality health and human services to the medically underserved and lowincome populations in a culturally sensitive manner.
  • Punctuality and Attendance: This is an essential job duty for CCCHC's employees given the impact on patients.
  • Other duties as assigned.

Special Knowledge, Skills, Abilities and Attributes:

  • Demonstrated ability to exercise sound judgment.
  • Ability to communicate verbally and in writing, clearly and concisely.
  • Ability to lead, plan and be organized.
  • Ability to work well under pressure, take initiative, be flexible and cooperative.
  • Ability to maintain confidentiality of patient information and Company records.
  • Ability to exhibit good interpersonal skills.
  • Good computer skills and ability to adapt to new software.
Education and Experience Requirements

  • High School Diploma/GED, or 13 years' experience
  • Experience in FQHC preferred.
  • Bilingual in English/Spanish required. (Other languages as necessary)
  • Physical Requirements
  • Occasional walking, reaching with hands and arms, stooping and kneeling.
  • Occasional lifting up to 25 pounds.

$ $20.26 per hour
