Overview:


Under the direction and guidance of the Youth Program Manager for Flagler Services and working closely with the Rapid Re-housing Team, this position is responsible for assisting young adults to move from homelessness into permanent housing.

This position will work closely with Flagler Management staff to help develop, implement and maintain the dimension of our Rapid Re-housing Program.

All work is performed in a collaborative, team based model with a systems approach. Work is performed safely, effectively, to the highest ethical standards, and in accordance with accrediting standards and St. Joseph's Villa's mission, values, and guiding principles.

This position is based in the Housing Resource Center in Petersburg.


Responsibilities:

Intake and Housing Search

  • Work with the Flagler staff to identify homeless youth/young adults and provide intakes in locations easily accessible to them
  • Work with the Housing Specialist to establish landlord relationships with landlords who are willing to rent to young adults who may have no or negative rental history
Assessment and Development of Stabilization Plan

  • Participate in interviews when feasible and/or meet with RRH team to assess applicant and determine housing barriers and develop initial rehousing plan
  • Work with client and other staff to develop the client's full plan for obtaining and maintaining his/her housing.
  • Assist client to create realistic goals with concrete steps which will ensure their housing stability
  • Coordinate temporary housing if needed
Move In Assistance

  • Meet with client as soon as possible to determine needs around utility deposits, furniture, moving assistance, etc
  • Assist client with coordination of move in activities such as trips to the SJV Donation Center or CARITAS to secure needed household items
Case Management

  • Schedule and conduct meetings with client to develop more comprehensive budget and to develop realistic Housing Stabilization goals.
  • Work with client on longer term stabilization goals which address employment/income, money management strategies and any other identified service needs.
  • Teach clients steps and communication tools to advocate for themselves in seeking and maintaining housing (including advocating for eligible benefits)
  • Refer client to other community resources to achieve goals and follow up to ensure referrals were completed.
  • Track and document client progress
  • Continually seek new community resources, such as health and medical services, food pantries, etc in areas where clients are residing
  • Use weekly Case Coordination meetings to share client progress and get support in working with high barrier clients.
  • Educate clients about RRH program, your role and their role.
Life Skills and Tenant Information

  • Work with Flagler Management staff to identify the right tools to assess and measure the youth's strengths, life skills and understanding of tenant rights and responsibilities
  • Implement these tools and work with Director of Operations to ensure Flagler has clear outcomes for this service (housing homeless youth) and is able to measure those outcomes
  • Work collaboratively with Youth specific community partners and SJV's Career and Transition Services (CATS) to support young adult clients attain education and employment
Documentation & Processing of Financial Assistance (subsidies, other)

  • Prepare check requests and invoices for financial assistance including utility deposits, rental subsidies and utility payments
  • Document and maintain up to date information on services provided and funds expended in the Homeless Client Information System (HCIS)
  • Place and maintain up to date grant mandated documentation in client hard copy file
Team Participation

  • Participate in staff meetings and other staff and group activities essential for the operation of an effective service.
  • Present information to team regarding client progress, lack of progress, with possible solutions to ensure best support for the client's success.
  • Promote good community relations and utilize community services and resources.
  • Attend scheduled training programs for professional development that includes, at a minimum, training required by St. Joseph's Villa and by regulatory and accrediting bodies.
  • Perform other duties as assigned.

Qualifications:

  • Bachelor's degree in human service field
  • Excellent communication skills and ability to work with a wide range of people parents experiencing the trauma of homelessness, landlords, government workers, etc
  • Able to read and interpret contracts
  • Ability to work some evenings and weekends if needed
  • Drivers License good driving record
  • Ability to use proprietary software

At least one year experience in the following:

  • Case management experience with adult clients
  • Experience/Understanding of issues affecting homeless families
  • Experience/Understanding working with domestic violence victims
  • Knowledge and practice usin
