The Coordinated Entry Lead/Youth System Navigator concentrates on the Waukesha County Coordinated Entry System, helping individuals and families navigate the coordinated entry system, ensuring that clients in need of housing services are properly assessed and prioritized on the housing queues.

The Coordinated Entry Lead/Youth System Navigator also exists as a point of contact to provide support and ensure compliance from partnering organizations that participate in coordinated entry.

The position provides a single point of contact for the Balance of State CoC within each of the YHDP participating locations.

Additionally, the Coordinated Entry Lead/Youth System Navigator helps homeless youth aged 18-24 identify housing needs and navigate complex systems within Waukesha County.

Services provided include needs assessments, case management, outreach, transportation support (as needed), and community referrals.

The position oversees a Youth Action Group in which homeless youth identify barriers within homeless services and advocate for change at the local and CoC level with support from the CoC staff.


Responsibilities:


  • Manage the Coordinated Entry process, ensuring participants qualify for homeless assistance services based on eligibility standards per the Department of Housing and Urban Development (HUD) and the Wisconsin Balance of State Continuum of Care Coordinated Entry guidelines.
  • Complete the Prescreen form to determine eligibility for further assessment.
  • Enter data/manage coordinated databases for those being prioritized for services.
  • Complete the appropriate assessments with all eligible people for homeless programming.
  • Manage the Non-HMIS data list for Waukesha County.
  • Run and review List Reports for accuracy every week.
  • Ensure that all missing or incorrect data is corrected in all data lists to ensure those with the highest priority are served in order.
  • Serve as the point of contact and collaborate with all Waukesha County school districts and their designated homeless liaison personnel to ensure coordinated entry is offered and conducted and streamline other supportive services.
  • Review, coordinate, and complete the required 30day followup with people with an open referral on the Prioritization List weekly.
  • Collaborate with all service providers participating in the Waukesha County Coordinated Entry System.
  • Ensure all participating providers complete and sign a Coordinated Entry Participating Staff Agreement and complete all the required Coordinated Entry training.
  • Contact clients on the priority list when housing program openings occur to start the admission process and record Entry/Exit in HMIS.
  • Introduce new participants to caseworkers and provide the program supervisor with all the required Coordinated Entry documentation to ensure a smooth program admission transition.
  • Work with the Coordinated Entry Committee to implement Waukesha's After-Hour Plan.
  • Document all efforts and contacts with clients to illustrate all movements on the Prioritization Lists.
  • Conduct regular reviews of participant files to ensure BOS, HUD, and EHH compliance.
  • Outreach to and serve as a liaison with community resources.
  • Maintain professional collaboration and communication with other community agencies.
  • Respect the right of confidentiality of participants, family, and caseworkers.
  • Adhere to organizational compliance with all local, state, and federal regulatory agencies.
  • Ensure compliance with all safety and work rules and regulations. Ensure the maintenance of departmental housekeeping standards.
  • Street outreach and inreach at congregate sites primarily focusing on homeless youth and families.
  • Outreach and market the services to youth in the community through social media, flyers, educational training for the community, and relationship building with providers who work with the youth and young adults experiencing or are at risk of homelessness.
  • Create a robust resource binder of all information about community resources that is accessible to all youth.
  • Facilitate training and discussion with stakeholders and other community partners around the unique needs and barriers experienced by youth in the community.
  • Communicate regularly with CoC staff about barriers and issues youth are having in accessing resources.
  • Advocate for change at the local coalition and CoC level with support from CoC staff.
  • Support the local Youth Advisory Board (YAB) by facilitating meetings, reviewing, and approving reimbursements, recruiting youth to participate in the local YAB, identifying additional leadership opportunities, and being the point of contact between the local YABs and the CoC Staff, specifically the Project Coordinator.
  • Ensure all services are provided through positive youth development, traumainformed, harm reduction, housing first lens.
  • Attend YHDP System Navigator meetings conducted by the CoC Project Coordinator.
