Description

The Intake Concierge & Wellness Coordinator is responsible for managing the stream of referrals to multiple agency or office locations.

The Intake Concierge & Wellness Coordinator is responsible for the overall operation's adherence to the Intake Policy.

The Intake Concierge & Wellness Coordinator will also be required to educate all personnel who accept referrals on the particulars of the Intake Process, the specifics associated with our clinical programs, products, and offerings including our customer service expectations associated with each call received.

The Intake Concierge & Wellness Coordinator will be required to initiate appropriate communication with clinical, operational, and sales teams based on the needs of referred patients and customers as outlined in the Intake Policy.


  • Follows all privacy policies of the organization and maintains the confidentiality of protected healthcare information (PHI).
  • Maintains the Referral paperwork/electronic records accurately.
  • Must be able to effectively communicate with all team members.
  • Educates team members on policy changes and new program offerings.
  • Articulates a working knowledge of clinical aspects of services provided to referral sources and potential patients.
  • Must be able to handle a variable workload based on referral patterns.
  • Receives Home Care referrals from MD's, Social Workers, Discharge Planners or other sources and assesses referred patients for potential homecare needs, including home equipment needs.
  • Will be a resource for referral sources in situations where the agency may not be able to meet the needs of the patient being referred.
  • Actively seeks to provide information regarding available home health services and specialty programs to patients, family and physicians.
  • Participates as needed in organization's Specialty Programs, including, but not limited to, the Admission Coordination Program and Patient Callback Program.
  • For payers that have patient financial liabilities, follows up with patient to determine method of payment, i.e. credit card, billing statement, or other arrangements.
  • Documents all supporting records in accordance with regulations of applicable regulatory agencies.
  • Informs sales and operational management aware of unmet community, referral source or patient needs so that appropriate services and programs may be developed.
  • Insures proper processing of information received from referral source to appropriate internal channel, including, but not limited to, field staff, sales team member, supervisory staff, billing department, and insurance verification.
  • Complete required clinical and monthly sales inservices to accrue needed continuing education units to maintain legal nursing status and expand knowledge base.
  • Remains flexible and open to new ideas when confronted with changes, creative brainstorming, and interpersonal skills for establishing treatment plans to meet individual patient needs and explore other options as appropriate.
  • Will participate in oncall rotation for Intake processing 24 hours a day, seven days a week as needed/requested by supervisor.
  • Maintains familiarity with all State and Federal Laws governing Medicarecertified Home Health and follows those laws appropriately.
  • Effectively cultivates new business opportunities within assigned territory and achieves net contribution growth goals necessary to support company objectives. Participates in effective networking activities as approved by the FL Region Director of Operations/General Manager.
  • Conducts client evaluations and coordinates postreferral client visits; compiles information needed to establish a definitive home care plan for clients including an assessment of the appropriateness of requested services and physician's orders.
  • Registered Nurse or LPN/LVN preferred.
  • Prefer a minimum of 2 years in home health.
  • Knowledge of Medicare coverage regulations, home health, and community resources.
  • Ability to work with people of various disciplines and from multiple locations.
