Responsibilities:
Program Overview
Extended Foster Care provides support to young adults to maintain eligibility for extended foster care services ensuring better outcomes as they transition into adulthood.
Extended Foster Care services includes case management, placement support, teaching life skills, and supporting clients in areas such as housing stability, employment, and education to achieve a successful transition into adulthood.
Position Overview
The EFC Success Coach engages youth in a coach-like way and provides strengths-based case management and interventions as needed for youth and young adults preparing for and participating in the Extended Foster Care Program.
- Meet with each youth/young adult at their place of residence a minimum of 30 minutes face to face 1 x per month (up to 3 x's as needed).
- Build a supportive relationship with youth/young adult that assists them in developing goals and a plan for achieving those goals. Monitor and adjust the plan with the youth based on progress and need areas.
- Supporting youth/young adults in crisis stabilization and identifying and addressing the root cause of the crisis. Providing interventions to address immediate safety until the young adult can start or continue in goal progression.
- Act as a liaison with community service providers. Coordinate with service providers to ensure that the youth/young adult's needs are being met while also collaborating with community service providers and other key stakeholders.
- Participate and/or facilitate teambased planning meetings at the invitation of the youth such as wraparound meetings, family engagement meetings, and permanency or community roundtables.
- Provide weekly support to youth/young adult by maintaining weekly contact and ongoing communication with service providers and other supports.
- Maintain ongoing development of a case/transition plan that reflects the young adult's personal goals.
- Complete required documentation including skills assessments, casenotes, transition plans and monthly progress updates within timelines outlined by the program.
- Utilize interventions to address the following domains: housing, education, natural supports/healthy relationships, employment, health, transportation, and money management.
- Conduct ongoing assessment of the young adults needs from a strength focused solutionbased perspective.
- Empower young adults to build upon and utilize support in their natural environment, including teaching young adults how to access the appropriate community resources.
- Participates in weekly supervision concerning case strategies and case progress.
- Act as an advocate for the young adult when facing barriers to achieving goals by attending meetings, and court hearings as determined by the youth, caseworker, or supervisor.
- 24/7 availability to support young adults, while rotating between staff, on call responsibilities for after hours, holidays, and weekends.
- Provide support accessing transportation for youth to include directly providing transportation to events as needed.
- Maintain a caseload of up to 20 young adults.
- Complete other duties as assigned by program leadership.

Additional Information
Schedule is flexible and non-traditional; based around the availability of youth and families served.
Applicants must have a valid driver's license, a personal vehicle to use for work purposes and auto insurance in the coverage amounts of 100/300/100.
Salary
$ $66000 / year based on education and clinical license

Requirements:
- Master's degree in a social services field is preferred; a Bachelor's degree in social services field is required.
