Position Summary:
Absolute Hospice Agency is seeking a compassionate and dedicated Medical Social Worker to join our team. As a Medical Social Worker, you will provide essential social work services to our hospice patients and their families. You will play a crucial role in addressing the emotional, psychosocial, and practical needs of individuals facing end-of-life care.

This position requires a strong commitment to providing exceptional care, excellent communication skills, and the ability to work collaboratively with a multidisciplinary team.


Responsibilities:

Conduct comprehensive psychosocial assessments of patients and their families to identify their needs, strengths, and resources.


Develop and implement individualized care plans in collaboration with the interdisciplinary team to address the emotional, social, and practical needs of patients and their families.


Provide counseling and support services to patients and their families to help them cope with the emotional challenges associated with end-of-life care.

Assist patients and families in navigating and accessing community resources, including financial assistance, legal services, and bereavement support.

Advocate for patients' rights and ensure their preferences and wishes are respected throughout the care process.

Collaborate with the interdisciplinary team, including physicians, nurses, chaplains, and other healthcare professionals, to ensure holistic and coordinated care.


Participate in interdisciplinary team meetings to provide input on patient care plans, share insights, and contribute to the overall care strategy.

Maintain accurate and up-to-date documentation of patient assessments, interventions, and progress in compliance with regulatory standards and agency policies.

Provide education and guidance to patients, families, and caregivers about end-of-life issues, grief, and bereavement.

Support and facilitate open communication and decision-making among patients, families, and the healthcare team.


Qualifications:
Current state licensure or certification as a Medical Social Worker.

Minimum of two years of experience in a healthcare setting, preferably in hospice or palliative care.

Strong knowledge of social work principles, bereavement support, and end-of-life care.

Excellent communication and interpersonal skills with the ability to empathize and support patients and families during challenging times.

Knowledge of community resources and the ability to assist patients and families in accessing appropriate services.

Ability to work independently as well as collaboratively within a multidisciplinary team.

Proficient computer skills and experience with electronic medical record (EMR) systems.

Compassionate, non-judgmental, and culturally sensitive approach to patient care.

Adherence to ethical standards and a commitment to maintaining confidentiality.
