    Job Description

    POSITION TITLE: House Manager – Community Living

    DIVISION: Community Living

    SUPERVISOR: QIDP

    CLASSIFICATION: Non-Exempt

    STATUS: Full-Time

    PURPOSE:

    Coordinates case management and provides personal, behavioral, and vocational counseling for adults participating in Adult Day Services.

    ESSENTIAL FUNCTIONS:

    1. Train staff in all programs, using lecture, role modeling, text, and other materials. Ensure client specific training is performed thoroughly.
    2. Attend and participate in quarterly and annual case conferences for clients in the habilitation programs with various locations and times, as needed.
    3. Participate in monthly meetings or as needed.
    4. Conduct Monthly interdisciplinary Team Meetings, as needed.
    5. Participate in monthly Case Record Review audits. Follow up as needed on missing data from files.
    6. Serve as a liaison between clients, families, habilitation staff, external professionals, and the community.
    7. Display patience in all situations involving clients. Speak in an even, positive, and personal tone of voice when conversing with clients.
    8. Appropriately respond to client's behavior via verbal and/or physical intervention based on the needs of the clients.
    9. Provide services in compliance with all funding sources and governing bodies (CARF, BQIS, State Board of Health, State and Federal Law, NISH & DOL).
    10. Follow policies/procedures set forth by the Community & Social Services Division and the agency.
    11. Conduct intake/discharge meetings and forms as needed.
    12. Provide requested information/records of adults formerly or currently served in Adult Day Services in compliance with agency HIPAA policies.
    13. Maintain confidentiality of all clients in compliance with agency HIPAA policies.
    14. Assist with departmental staff training needs.
    15. Provide direct care as needed and participate in the on-call rotation for the Community Living Departments.
    16. Respond to calls or tasks as requested for issues that could arise after hours or on weekends.
    17. Conduct CSTs and orientation trainings as needed.
    18. Complete BDDS incident reports and investigations, as needed, or as directed from supervisor.
    19. Work with E & E Coordinator to ensure curriculum and community integration guidelines are met as specified in current and ongoing HCBS regulations.

    OTHER FUNCTIONS:

    1. Attend staff/agency meetings and training sessions as authorized.
    2. Assist in the coordination of ancillary services for clients as needed.
    3. Provide resource information, assess, address service needs, and provide referrals to appropriate community agencies and internal departments, as needed.
    4. Maintain a customer and a co-worker satisfaction level that limits adverse situations or complaints.
    5. Attend/request conferences, workshops, or other training sessions to continually expand knowledge and upgrade skills related to job duties.
    6. Must be able to care for the physical needs of persons served including feeding, hygiene, and toileting.
    7. Must be capable of assisting in physically transferring/moving persons served when needed. Physical activities may include but not be limited to: lifting up 25-50 pounds (depending on person served);
    8. bending; reaching; stretching; grasping; visual acuity; walking; squatting; twisting; guiding; and transporting.
    9. Use of medical adaptive equipment including, but not limited to: Wheelchair, Hoyer Lift, Shower Chair, Blood Sugar Testing Equipment is required.

    ENVIRONMENT:

    • Occasional high noise levels may require the use of hearing protection.
    • Occasional exposure to fumes, oils, paints, and other industrial odors.
    • Potential exposure to bodily fluids, pests, contagious pathogens, and/or bed bugs.
    • Must be able to climb stairs on an occasional basis to access work areas, which are currently without elevator access.

    PERSONAL DEVELOPMENT:

    Attend/request related trainings as authorized or required.

    QUALIFICATIONS:

    1. Experience in DD, education, behavioral science, or related field preferred. High School diploma required.
    2. Previous experience working with persons with intellectual disabilities preferred.
    3. Must possess excellent interpersonal skills to meet the needs of those served.
    4. Must possess excellent communication skills both written and oral, and strong teamwork skills.
    5. Proficiency in Microsoft Word and Excel.
    6. Must possess or be able to obtain CPR and first aid certification annually and perform as necessary.
    7. Must possess or be able to obtain CPI certification annually and perform as necessary.
    8. Possess and maintain a clear criminal record per Medicaid Waiver regulation requirements.
    9. Maintain a valid Indiana driver's license, insurance on personal vehicle (minimum of PLPD with Medical liability coverage), and maintain insurability as defined by the agency liability insurance carrier.
    10. Must possess and maintain reliable transportation that meets state requirements for transporting persons served.
    11. Must have the ability to successfully work in both independent and group settings. Must be capable of communicating effectively during a crisis situation.
    12. Dress appropriately as defined by department dress code.
