Summary:
Essential Duties and Responsibilities:
- Provide individualized support and advocacy for residents, assessing their needs, strengths, and challenges.
- Develop and implement service plans, collaborating with the youth, their families (if appropriate), advocates and relevant community resources; ensure that the case file is up to date.
- Offer trauma informed crisis intervention and emotional support to youth experiencing difficulties or facing challenging situations.
- Regularly review and assess the effectiveness of service plans, making adjustments as needed to ensure the best outcomes for the youth.
- Collaborate with the interdisciplinary team at YES to coordinate services and support for the youth.
- Cultivate and maintain relationships with external agencies, community partners, and service providers to expand the resources available to YES youth.
- Stay informed about community resources and programs that can benefit the youth and their families.
- Organize and/or facilitate workshops or support groups for youth to enhance life skills, educational opportunities, and employability.
Competencies:
- Educate and counsel residents to work through issues they face, to include, but not limited to: trauma, substance abuse, highrisk sexual behaviors, familyrelated issues, and social/emotional development.
- Active listening open to listening to residents and employ deescalation techniques to resolve conflicts.
- Facilitate experiential learning workshops.
- Receive and process intake referrals for program admission.
- Complete intake packet for admission including Emergency Placement Plan.
- Act as agency liaison with assigned case manager from referring entities.
- Coordinate the services for approximately residents.
- Collaborate with Youth Care Workers around resident's daily living activities.
- Coordinate efforts so that the basic needs of the residents are met (i.e. medical, dental, psychological, psychiatric, educational services and appointments)
- Escort resident to court hearings and other mandated meetings as required.
- Document all resident related activities and complete all required reports.
- Attend all advocacy meetings regarding resident.
- Implementation of the "YES Values" doctrine into performance and practice.
- Attend and participate in required trainings, other agency trainings, case conferences, and other applicable agency meetings.
- Adhere to Agency's code of ethics and all aspects of the PA 3800 regulations.
Required Education and Experience:
- Bachelor's Degree in Human Services or comparable field.
- Minimum of three years' work experience with adolescents
Work Environment:
Physical Demands:
The employee must occasionally lift and/or move up to 30 pounds.
