Position Summary:
Responsible for providing case management services to Choice Neighborhoods residents to support their advancement toward economic self-sufficiency and improved quality of life, particularly in achieving the self-sufficiency outcomes outlined in the Choice Neighborhoods People Strategy and residents' Individual Development Plans.
Case management services include outreach; eligibility determination; assessments; individual development planning; referrals to supportive services providers; service coordination; ongoing monitoring, coaching, and counseling; retention; and tracking achievement of outcomes.
Essential Duties and Responsibilities:
Under the direction of the Manager, Supportive Services, works collaboratively with company's Communications, External Affairs, and Choice team members on the development and implementation of outreach strategies.
Manages a caseload of approximately 50 households annually delivering case management services to Choice Neighborhoods residents that includes the following:Conduct supportive services eligibility determinations on interested residents.
Assess clients' social services needs using tools provided by company and assist clients with Individual Development Plans.
Refer clients to supportive services providers and other organizations offering services that meet client needs.
Monitor, track, and conduct follow-up on client participation in programs and services and progress in achieving goals and outcomes.
Use company's designated database system, to record client demographics, case management activities, referral and participation status, and progress consistently and accurately in achieving goals and outcomesCoach and counsel clients to support service retention and goal achievementSupport the Senior Case Manager in connecting youth and their parents to educational services and resources that that seeks to improve educational outcomes and intergenerational mobility; andMeet with services providers, in coordination with manager, to review resident participation, progress, outcomes, and to address issues or concerns.
Ensure that in the delivery of case management services, there is a streamlined and strong focus on achieving the overall goal of the Choice Neighborhoods People Strategy:of "delivering services and supports to improve resident workforce, educational and health outcomes with a special focus on youth and their families.
Educate clients on how to access community-based supportive services on an ongoing basis.Work collaboratively with Manager and Data Analyst to validate case management data and reporting to ensure accuracy.
Achieve, maintain, and communicate expert knowledge on community-based service provider resources, the services they offer, and eligibility requirements, to ensure accuracy and effectiveness in referring clients to services that meet their needs.
Effectively achieve Key Performance Indicator (KPI) metrics as defined by Manager.Perform other duties as assigned.
Education and Experience:
Bachelor's degree in social work or a related field required. Four (4) years of case management experience to include experience working with low-income individuals and/or families.
Knowledge and Skill Requirements:
Ability to assess client human development needs and develop Individual Development Plans in collaboration with clients.
Demonstrated ability to manage up to 50 households' caseloads with varying issues and challenges.
Demonstrated ability to utilize Microsoft Office suite (Outlook, Word, Excel) and case management technology to prepare and maintain accurate, comprehensive, and up-to-date information on client caseload activity.
Must exhibit good judgment, tact, decision-making and diplomacy when dealing with internal and external customers and confidentiality of resident information.
Strong verbal and written communication skills and being a team player with ability to interact with staff, residents, and providers in a professional manner delivering complete and accurate information.
Demonstrated ability to plan, organize, and prioritize efficiently in an environment of multiple and shifting priorities.Ability to work independently and fully execute assignments without intensive supervision.
Working Conditions:
Works primarily in an office environment. May engage in frequent evening community-based meetings for client outreach and community engagement. May require some bending, stooping, reaching and prolonged sitting. May require some lifting usually not exceeding 10 lbs.
