***Responsible for community-based direct and indirect care given to assigned Class Members in compliance with state regulations, professional standards of practice, physician orders, and individual health/treatment plans. For pre-transition, conducts nursing assessments, completes documentation, and participates in external reporting for UIC stuffings/consultations regarding health needs of Class Members. Attends team meetings, collaborates with other staff, and prepares case files for CAST calls. For post-transition, carries a nursing services caseload, as well as provides follow up care and monitoring subsequent to reportable incidents, hospital discharges, and ER visits. The ability to rapidly gather information, assess, and problem solve in a fast-paced, changing environment. Administers and monitors prescribed medications and treatments in accordance with approved nursing techniques and physician's orders. Provides case management to persons with severe mental illness or co-occurring disorders, both on-site and in the community. Recommends and refers Class Members to appropriate medical services. Documents all services using program standards. Acts as a collaborative member of an interdisciplinary team in agency and participates in risk mitigation planning. Provides training to staff and back-up coverage to other agency nurses.


REQUIREMENTS

  • Nursing degree from accredited university/college.
  • Experience working with persons with psychiatric disorders and dualdiagnosis population required.
  • Valid Illinois Registered Nurse (RN) license required.
  • Valid Illinois driver's license, insurance, and daily access to vehicle.
