Role description


GENERAL JOB DESCRIPTION:
Under general supervision, the 2830 Public Health Nurse performs professional nursing duties.
Essential duties include but are not limited to all or some of the following:

Offers public health nursing services in maternal and child health; conducts home visits for expectant mothers; interprets medical orders and supports mothers; provides guidance on nutritional needs, hygiene, and safety measures; advises post-delivery on postpartum care and obtaining emergency medical assistance; examines and monitors the physical condition and development of newborns and assists new mothers in infant care.

Delivers nursing services directly to the elderly, infirm, chronically ill, mentally ill, emotionally disturbed individuals or families, and people experiencing homelessness; arranges for necessary medical care; refers cases to suitable welfare and social agencies; oversees cases and maintains records of nursing and other services provided.

Offers nursing services, care, investigation, or consultation related to controlling communicable diseases and outbreaks; conducts interviews with cases and those suspected of active infections at their homes, assigned unit/area, health center, or over the phone; interprets medical advice regarding care and treatment and guides patients on self-assistance.

Utilizes community organization concepts to address community health needs, and directs patients and families to the appropriate agency for financial assistance, casework services, and other human services to ensure ongoing patient care.

Conducts case finding in various settings, keeps records of visits and services, and shares this information with medical and administrative staff.

Utilizes Electronic Health Record and other documentation systems to create and manage individual case records for patients and cases; employs related forms for referrals, consent reports, narratives, statistical records, and other purposes; compiles various statistical information like clinic attendance, tests, immunizations, and other relevant records.

Supports health promotion/risk reduction intervention and offers health teaching, screening, advocacy, and counseling to clients of all ages, including high-risk populations.

Keeps current, clear records in line with legal guidelines.
The 2830 Public Health Nurse performs other duties as assigned/required.

How to qualify


LICENSE:
Possession of a valid permanent/temporary California license as a Registered Nurse issued by the California Board of Registered Nursing; AND

CERTIFICATE:
Possession of a valid Public Health Nurse Certificate issued by the California State Department of Health; AND

EXPERIENCE:
One (1) year (equivalent to 2,000 hours*) of verifiable experience as a Registered Nurse in an acute hospital, primary care facility, home health agency, and/or public health agency within the last five (5) years; AND

LICENSE:
Possession of a valid current California Driver's License issued by the California Department of Motor Vehicles. (Possession of a valid California Driver's License is required at the time of appointment.)
Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted.

Note on Experience:

One year of full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week).

Any overtime hours that you work above 40 hours per week are not included in the calculation to determine full-time employment.
