***: The Service Coordinator is responsible for the direct coordination of services and supports to residents living in the specified supportive housing. They are responsible to identify and assess needs and access resources beneficial to residents as needed and to act as an advocate on the residents' behalf.


ESSENTIAL DUTIES AND RESPONSIBILITIES:


  • Meets with new residents to explain the Service Coordinator's role in providing information and support in assisting residents interested in accessing local services resources. Provides needed support for those residents moving out of the housing site.
  • Develops supportive-professional relationship with residents that help them enhance the quality of their lives.
  • Provides supportive linkage between residents and WestMass ElderCare.
  • Coordinates and promotes educational, health (wellness), and recreational programming by creating newsletters, calendars, and/or information pertaining to these events. Ensure that all materials are distributed in prevalent languages and large font and/or other needed formats.
  • Identifies and assesses individuals needs as needed, informs the resident of available resources including ADH, (G)AFC, Home Care, One Care, PACE, Personal Care Management, SCO, MassHealth/Transitional Assistance, SNAP, Veteran services and benefits, Waiver, and provides support in accessing other needed community resources such as the local Senior Center, DDS, DMH, and MRC.
  • Provides appropriate assessment and referrals for all ensure that they have access to appropriate community resources.
  • Identifies gaps in services for residents and is knowledgeable about the services needed by all residents.
  • Provides leadership and service coordination with community resources and the community network in which resident resides.
  • Coordinates the work of provider agency staff and 24/7 emergency coverage and acts as liaison with the Resident, Provider Agency, and the Housing Authority.
  • Assist with providing needed oversight and support for the on-site congregate meals program.
  • Participates and/or facilitates care conferences with residents, family, and other participants to educate about service needs, provides and solicits advice about specific resident issues and resources and resolves conflicts.
  • Completes, maintains and keeps current, comprehensive case records according to established procedures and time frames per EOEA documentation standards for each resident/consumer. This includes completing and keeping current resident/consumer records in the required data base.
  • Maintains and submit all required information in a timely, organized, accurate and confidential manner.
  • Attends relevant internal and external meetings, in services and training programs as designated.
  • Develops and maintains an effective relationship with designated housing site management and community service organizations in order to ensure appropriate services for residents (this includes the management staff, the maintenance staff and community providers).
  • Attends interdisciplinary team meetings, in-services, and relevant outside training programs as designated. Collaborates with designated staff to insure continuity of care and service for residents transitioning to another setting
  • Responsible for tracking and compiling statistics needed for monthly board report and EOEA Supportive Housing
Quarterly statistical report, Annual Narrative Reports and other reports/assignments requested by EOEA.


  • Facilitates the distribution and collection of annual housing surveys per EOEA guidance and provides resident updates of the upcoming surveys.
  • Provides advocacy, conflict resolution, and crisis intervention/support for residents in need.
  • Participates in outreach activities and events which promote supportive housing and service options at the designated site
  • Maintains regular on-site office hours (minimum of 20 hours), for sites that have multiple buildings, the RSC shall be onsite a minimum of 20 hours/week total and a minimum of 3 days with hours split in each building and accepts other assignments and responsibilities within the scope of this position as assigned by Supervisor.
  • Disseminates RSC office hours with emergency contacts and maintenance information to all residents.
