General Responsibilities


The Registered Nurse, Bronx Assertive Community Treatment (ACT) Older Adult Services, is responsible for conducting assessments, assessing basic health and medical needs, coordinating healthcare with community medical providers, providing management and administration of medication in conjunction with the psychiatrist/NPP, providing a range of treatment, rehabilitation, education and support services.


Primary Responsibilities

  • Conduct comprehensive assessments of clients' mental health needs and develop individualized care plans.
  • Collaborate with the ACT Team to deliver evidencebased nursing interventions and treatments.
  • Monitor and administer psychiatric medications, ensuring compliance and addressing any side effects.
  • Provide education and support to clients and their families on mental health issues, treatment options, and wellness strategies.
  • Participate in crisis intervention and deescalation as needed, ensuring the safety and wellbeing of clients.
  • Coordinate with community resources, including primary care providers, social services, and housing agencies, to enhance the continuum of care.
  • Document client interactions, treatment plans, and progress notes in accordance with organizational policies and legal requirements.
  • Actively participate in team meetings, case conferences, and supervision sessions.
  • Additional tasks as assigned

QUALIFICATIONS:


  • Current license and registration to practice as a Registered Nurse in NYS.
  • Must be computer literate, including ability to learn and use programspecific software.
  • Experience with older adults and/or mental health population preferred.
  • Willing and able to work a flexible schedule which may include weekends and holidays.

  • Friday, 9AM5PM
35 Hours Per Week
