HOSPICE RN CASE MANAGER

Comfort 1 Hospice is a homegrown team of local healthcare providers who have come together for our community to provide comfort and compassion to those with life-limiting illness.
Responsibilities:
The RN Case Manager is an active participant of the interdisciplinary group and also works in collaboration with all team disciplines.
As the RN Case Manager, you will be creating, implementing, and evaluating a patient driven care plan that is specific for each patient and their family.
Some additional responsibilities include completing head-to-toe physical assessments, reporting findings to the Medical Director or Attending Physician, maintaining accurate and up-to-date medical records, and performing supervisory visits for the Hospice Aide and LPN when applicable.
Job Requirements
- Must have current RN license to practice as an RN in the state of Indiana
- Must have at least 1 year of Hospice Nursing experience
- Must have strong written and verbal communication skills
- Must have the ability to perform the physical duties of the position; including having to bend, stoop and stand for extended periods of time, and also be able to lift at least 50 pounds.
- Must have reliable transportation, current auto insurance, and driver's license
