- Participates with the Administrator and COO in the formulation of a business plan, i.e., strategic goals, resource allocation plan(s) and a yearly budget.
- Policy formulation and programmatic development and implementation including assessing adequate and appropriate staffing.
- Coordinates planning activities to develop administrative policies and practices for the Agency.
- Consults with supervisors and staff, individually or in groups, regarding patients or special programs or service programs.
- Coordinate, and schedule, the daily workload of each district, utilizing Agency personnel to their fullest capacity.
- Oversees all CQI and utilization review activities.
- Assists in Agency evaluation activities.
- Coordinates with other program areas and senior management
- Acts as liaison with Staff Development for program areas.
- Assesses appropriate staff supervision during all service hours.
- Collaborates in the development of an effective and efficient patient care documentation system
- Participates in selected orientation programs.
- Directs staff in performance of their duties.
- Provide daily supervision of district nurses, and Home Health Aides
- Review, and investigate, the clinical content, diagnoses, medical history, medications, fee status, and other procedures
- Direct staff toward the appropriate use of community resources.
- Appraise staff performance, during their probationary period, and annually
- Identify, and evaluate, patient needs and appropriateness of home care
- Participate in community related functions, as necessary.
- Assist with the orientation of new professional staff
- See patients in the home setting, as needed.
- Participate in audits, U.R. interdisciplinary team conferences
- Review each nurse's schedule for recertification, discharges, and Aide supervisory visits.
- Review weekly schedule submitted by nurses for changes or updates.
- Fill out the patient's re-hospitalization referral/admission form sheet, daily
- Perform other duties as directed, by the Administrator or Clinical Manager.
- May be requested, to fill in for the other supervisory staff, when necessary.
- Bachelors degree in Nursing required but Masters degree preferred.
- Graduate of NLN accredited school of Nursing
- Licensed RN through the Massachusetts State Board of Nursing
- 5+ years of clinically related experience in home healthcare setting
- Bi-Lingual RN in Spanish preferred
- Licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
