The Opportunity:
We are currently seeking a compassionate and dedicated Case Coordinator - Senior Care Specialist to join our team.

In this role, you will be responsible for providing comprehensive case management services to seniors and their families, ensuring that their unique needs and preferences are met with professionalism and compassion.


What You'll Do:


  • Collaborate with clients and their families to assess needs and develop personalized care plans tailored to their individual preferences and goals.
  • Coordinate a wide range of home care services, including caregiver assignments, medical appointments, and transportation, to ensure seamless delivery of care.
  • Serve as a trusted advocate and liaison between clients, caregivers, healthcare professionals, and other stakeholders, facilitating open communication and collaboration.
  • Monitor clients' progress and adjust care plans as needed to meet changing needs and preferences, ensuring that they receive the highest quality of care.
  • Provide ongoing support and guidance to clients and their families, addressing concerns and resolving issues with empathy and professionalism.
  • Maintain accurate and uptodate documentation of client information, care plans, and service delivery, in compliance with agency policies and regulations.

What We're Looking For:


  • A compassionate and empathetic individual who is passionate about serving seniors and improving their quality of life.
  • Previous experience in case management, care coordination, or a related field, preferably in a home care or healthcare setting.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients, families, and caregivers.
  • Strong organizational skills and attention to detail, with the ability to manage multiple tasks and priorities effectively.
  • Proficiency in Microsoft Office365 and Salesforce systems preferred.
  • Valid driver's license and reliable transportation required.

$ $24.00 per hour
