Description
Obtain, review and file necessary paperwork, including: patient medical records, state/federal mandated paperwork, admission forms, etc.- Upload information into EMRs.
- Provide information to residents & staff by answering questions and requests.
- Maintain resident confidence by monitoring confidential information processing.
- Maintain a cooperative relationship among health care teams.
- Establishes the patient's eligibility for postacute care in collaboration with discharging facility.
- Interacts with a variety of hospitals social service/case management staff regarding patients discharge plans.
- Assess clinical status of patients upon hospital admission referral to establish suitability of prospective admissions.
- Inquire about clinical status of current Residents upon hospital admission to ensure a smooth transition for return to facility.
- Reviews patients' medical records.
- Meet and communicate postacute care to patients and their families as needed.
- Coordinates with healthcare facility staff to ensure that each patient has a smooth transition through the admission process.
- Verifies health insurance coverage for patients with discharging facility to assistance and support.
- Working knowledge of major insurance, Medicaid and Medicare requirements for reimbursement.
- Develops relationships in the community on behalf of the postacute care facilities.
- Works with the facility's marketing and sales team to bring awareness of the facility's strengths and services into the community at large.
- Follow patient from hospital to skilled nursing location and documents final disposition or reason for not admitting, case load includes patients discharged from Van Rensselaer back to the hospital.
- Obtain new high acuity inquiries and assesses special patients to evaluate care needs prior to admission, and directs to appropriate center.
- Evaluate inquiries and communicates information to case manager, and facilitates patient's smooth transition into center.
- Obtain accurate information from physicians, patients, and referral sources regarding the expected discharge plan, and communicates this information to facility as needed.
- Cultivate relationships with sources to generate repeat referrals.
- Participating in facility's marketing education, ensuring knowledge of their resources and maintaining knowledge of competitor facilities.
- Providing facility tours and conducting presentations for potential referral sources and patient families.
- Attending seminars and conferences to promote facility.
- Communicating activities and concerns to supervision.
- Completing required paperwork, managing correspondence and maintaining data as required.
Pay:
From $67,375.00 per year

- Admissions: 1 year (preferred)
