Job DescriptionThis individual fulfilling this position shall be responsible directly to the HCH Director. The individual fulfilling this position shall have at least a degree in Nursing and be licensed as a Nurse by the State of Missouri Dept. of Economic Development, Division of Professional Registration.HOURS AND LOCATION OF SERVICE If the individual fulfilling this position is employed by the Clark Community Mental Health Center on a full time basis, the work week shall consist of up to 40 hours; possible after hours may be required. The person fulfilling this position will receive regular staff benefits as outlined in the Personnel Policy.
MAJOR AREAS OF RESPONSIBILITY FOR THE POSITION OF NURSE CARE MANAGER1. INFORMATION/EDUCATION - The individual fulfilling this position may be responsible for providing Information and Education services. This may involve making community presentations and providing direct consultation to other service providers. Primarily training on medical diseases, treatments, and medications. 2. Develop wellness and prevention initiatives. 3. Facilitates health education groups. 4. Participates in the initial treatment plan development for all Healthcare Home enrollees. 5. Consult with Care Coordinators about identified health conditions. 6. Assist in contacting medical providers and hospitals for admission/discharge. 7. Track required assessments and screenings. 8. Assist in implementing DMH Net health technology programs and initiatives. Cyber Access, Care Manager, and Metabolic Screenings.9. Monitor HIT tools and reports for treatment and medication alerts and hospital admission/discharges.10. Monitor and report performance measures and outcomes.11. Participate in working with clients who have a goal of tobacco cessation as appropriate.
SUPERVISION RESPONSIBILITY a. The individual fulfilling this position may be required to provide programmatic supervision to other staff as assigned by the immediate supervisor.b. The individual fulfilling this position is expected to receive and seek supervision to successfully meet the responsibility of this position.
DOCUMENTATION RESPONSIBILITY - The individual fulfilling this position will be required to complete all necessary documentation in a timely manner.
OTHER DUTIES - Other duties may be assigned by the Program Director or the Chief Executive Officer.
