Social worker or health and nursing professional (m/f/d) Social Services / Discharge Management Sana Clinics Duisburg We are looking for a social worker or health and nursing professional (m/f/d) to join our Social Services / Discharge Management team at our acute care hospital in Duisburg.

This position is available immediately and is full-time (38.5 hours/week) and permanent.

Sana Clinics Duisburg GmbH operates two hospitals:
Wedau Clinics in Duisburg-Wedau and Bertha Hospital in Duisburg-Rheinhausen. Both clinics are modern teaching hospitals with a total of 17 departments and 678 beds, providing regional top-level care.

Your responsibilities will include:
Providing advice, information, and support to patients, caregivers, and family members in social, economic, rehabilitative, vocational, and/or nursing matters Collaborating with an interdisciplinary team You are: A graduate of a social work program (diploma or bachelor's degree) or a successful graduate of a health and nursing program Knowledgeable about social legislation, proficient in computer skills, and have experience in the healthcare field Customer-oriented, able to coordinate and work in a team, and have strong communication skills, empathy, and reflective abilities Willing to work cooperatively with other professional groups Possessing high professional, personal, and social competence Able to handle stress, reliable, flexible, and a team player What we offer: Variety | An interesting, diverse, and responsible position Competitive salary
