Early Intervention Service Coordinator

JOB LOCATION**:
CFC #1- Champaign County


POSITION TYPE:
Full-Time (40 hours/week), Work From Home but must live in Champaign County, Illinois


Child and Family Connections of Central Illinois is the regional intake agency for children birth to three and their families to enter the Illinois Early Intervention System.

Early Intervention (EI) credentialed Service Coordinators work with families to develop an Individualized Family Service Plan and select service providers to meet the individualized needs of their child.

Child and Family Connections can also assist with Social Emotional Consultative Services and free developmental screenings through the Local Interagency Councils.


Qualifications

  • Bachelors degree or higher from an accredited college or university in Education, Social Work, Human Services, Behavioral Science, or a healthrelated field
  • Official Transcript
  • Eligible for Early Intervention Service Coordination Credential
  • Proven successful experience working with children and families preferred

JOB DESCRIPTION:

Service Coordinators support families living in the geographically assigned area by assuring all new and eligible children with a possible developmental delay and their family receive the services they need by:

  • Informing them of their rights and services under the Early Intervention Program, assuring that all materials are communicated in their primary language and that their confidentiality is maintained;
  • Collaborating efforts amongst providers if a family is receiving services from other private or public agencies;
  • Facilitating and participating in the development, review, and evaluation of the Individualized Family Service Plan (IFSP);
  • Providing information on service delivery options in the child's natural environment including the child's home, daycare or other environment deemed appropriate;
  • Helping families select an evaluator and arranging the initial evaluation;
  • Coordinating evaluations and assessments;
  • Coordinating and monitoring the delivery of services;
  • Assisting the family with transition to their appropriate school district for children nearing age 3.
