Job DescriptionSummary of Work: With the support of the assigned Program Manager, the Recovery Care Coordinator (RCC) is responsible for coordinating and providing care for clients struggling with drug and/or alcohol addiction. The RCC provides primarily home and community-based case management services that focus on building relapse-prevention skills, increasing recovery capital, and creating self-sufficiency. The RCC will develop a supportive relationship with individuals and provide consultation and care coordination with varying supportive agencies, including transitional housing, hospitals, mental health agencies, substance use programs, and other recovery supports within LSSI and the community. They will also provide connections and referrals to individuals and families in need of supportive services and complete appropriate documentation within established timeframes. The RCC is responsible for conducting required screenings and assessments, writing case plans, and providing case management and care coordination according to department needs, guidelines, and procedures.
Essential Duties and Responsibilities:
•Effectively engages individuals and families in the therapeutic process and completes mental health/substance use assessments as appropriate, utilizing a systems framework and trauma- informed approach among other modalities.
•Works independently and consistently exercises discretion and sound judgment in providing short and long-term case coordination as well as stabilization and support services to individuals and families needing specialized intensive care coordination and behavioral health services; meets programmatic benchmarks for productivity.
•Respects the policies and procedures of all community agencies when conducting screenings, assessments, appointments, and one-on-one visits.
•Delivers services in the home and community-based settings and adapts to the unique requirements of each setting.
•Responds to client crisis on evenings and weekends as appropriate.
•Transports clients as needed in personal vehicle.
•Responsible for thorough, timely, and accurately written documentation/reports for the client file and funding.
•Completes paperwork and follows agency procedures and accreditation guidelines while performing work duties. Helps with reaccreditation process when assigned.
•Records all necessary information in the prescribed data systems.
•Completes statistical information in a timely manner for agency and funder purposes as required.
•May co-facilitate or lead support groups, conduct psychosocial educational groups, and provide presentations to community groups or consultations to schools and secondary settings. May provide recommendations and testimony for court (specifically when working with DCS-involved clients).
•Assists in providing referral information to families and connects individual/family to additional services as needed if the program is full, or they do not meet the minimum program requirements at LSSI.
•Develops and maintains working relationships with community partners to educate referral sources on Recovery Care services, collaborate, and establish an ongoing dialogue.
•Serves as an advocate for the client while respecting all confidentiality policies across varying agencies, programs, and meeting locations.
•Offers and provides evidence-based curriculum and education to clients as appropriate.
•Meets with Supervisor on a regular basis and consults with Supervisor on assigned cases.
•Reports any case that may involve "imminent harm" or "threatened harm" to a supervisor
immediately. Always intervenes on behalf of the child when necessary and reports to the Department of Child Services in cases where neglect and/or abuse is suspected.
•Attends staff meetings, in-service trainings, and other meetings as requested.
•Attends department team/staff meetings and presents cases, as requested.
•Attends program-related meetings in the community, as requested.
•Tracks balanced score card goals and turns in required reports to the program manager.
•Provides assistance in other programs, counties, and regions when necessary.
•Performs other related duties as assigned.
•Participates in grant writing and subsequent reporting for programs.
•May assist Program Manager with training of new staff.

•Knowledge of human behavior and dynamics. Demonstrates maturity and experience in successfully working with people of all ages, families, and those struggling with addiction.

•Knowledge of or ability to learn community resources appropriate to needs of clients.

•Knowledge of or ability to learn program policies and procedures.
•Understanding of and empathy for clients served, including those with differing cultural, racial, or ethnic identity.
•Attention to detail and ability to multi-task with strength in organizational skills.
Education and Experience:
•Bachelor's degree in social services or related field preferred.
•Certified Addiction Peer Recovery Coach or Certified Peer Support Professional (previously CHW/CRS) preferred.
•Minimum of two years' experience working with addiction/recovery preferred.
•Unencumbered Licensed Addiction Counselor (LAC) in the State of Indiana preferred, or CADAC, NAADAC, or LAC eligible accepted.
•Experience in screening, assessments, treatment and recovery strategies, Motivational Interviewing, and strategies for working with child welfare agencies, correctional institutions and other health and social services agencies that serve individuals with substance use disorders.
•Valid driver's license and car for full-time business use.
Minimum Auto Insurance Coverage Levels:
Bodily Injury Liability
•$100,000 for any one person in any one accident
•$300,000 for all persons in any one accident
Property Damage Liability
•$100,000 for any one accident
