What will I do as a Hoarding Case Manager?

You will provide case management services for clients in the Hoarding program, conduct the initial assessments to develop the client's care plan, and regularly partner with Home Coaches, Hoarding Interventionists, and Therapists to make sure the program is helping the client in the most efficient way possible.

Within this role, you will serve as support to both clients and hoarding staff and ensure the quality of service of the Hoarding Intervention and Treatment program.


  • Develop, in collaboration with the Home Coach and Therapist, individualized care plans for the clients.
  • Conduct client inquiries and in home assessments to identify clients physical, social and financial wellbeing needs.
  • Help clients sign up for services such as Medicare, food stamps, family care, disability etc.
  • Monitor cases by observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support
  • Maintain client's records by reviewing case notes; logging events, progress and reports.

Salary:
$38,000 - $40,000

Schedule:
Monday through Friday, 8:30am to 5:00pm


Qualifications:


  • Bachelor Degree in Social Work, Psychology, Sociology, or related field
  • Two years of previous experience working with older adults, people with disabilities, and a variety of cognitive, physical, and mental illnesses is a plus.
  • Valid driver's license, reliable transportation and vehicle insurance (liability coverage: $100,000/person and $300,000/accident).
