    CenterWell Senior Care Centers* is seeking a Board Certified/Board Eligible Advanced Practice Provider for a position in Mesa, AZ*.

    *As a Provider with CenterWell Senior Care Centers, you'll have the opportunity take a hands-on approach to addressing patient needs inside the exam room, as well as looking beyond to understand how social determinates of health are impacting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a multidisciplinary approach.
    *Thrive in a value-base care model and improve patient's quality of life

    • See a variety in your caseload, from preventative care to chronic conditions
    • Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care
    *This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. Familiar with Value Based Care model

    • Outpatient Senior Care Experience
    • Familiar with HEDIS measures
