- We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at our rehabilitation facility.
- As an LTC LPN, you will play a crucial role in assisting residents with their healthcare needs, administering medications, monitoring vital signs, and collaborating with other healthcare professionals to ensure the well-being of our residents.
- Provide direct nursing care to residents in accordance with established care plans, policies, and procedures.
- Administer medications and treatments as prescribed by physicians and monitor residents for any adverse reactions.
- Monitor residents' vital signs and assess their overall health status regularly.
- Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans for residents.
- Document residents' care and progress accurately and timely in electronic medical records (EMR) systems.
- Monitor residents' nutritional status and assist with feeding as necessary.
- Graduate of an Accredited Nursing Program
- Minimum of 1-2 years of experience in long-term care or rehabilitation settings preferred.
- Valid license as a Licensed Practical Nurse (LPN) in NY
- BLS– Basic Life Support
