    Case Manager Position at Terros Health

    Terros Health is excited to offer a fulfilling opportunity for a Case Manager at our 27th Avenue Certified Community Behavioral Health Clinic (CCBHC). As a Case Manager, you will be responsible for working with individuals diagnosed with General Mental Health conditions. Your role involves managing clients receiving medication only, conducting assessments, crisis plans, and individualized service plans. You will also be documenting client progress and creating a safe environment for recovery.

    Our programs at Terros Health are comprehensive and holistic, covering primary care, nutrition, counseling, addiction treatment, and more. The Outpatient program aims to help clients achieve recovery, improve their quality of life, and enhance their integration within the community.

    Responsibilities:

    • Collaborate with a clinical team to support clients with General Mental Health diagnoses
    • Educate, engage, and coordinate care for patients on medication services
    • Conduct outreach, crisis interventions, and follow-ups to ensure client well-being
    • Coordinate care with families, medical providers, and other support systems
    • Provide exceptional customer service to various stakeholders
    • Evaluate treatment effectiveness through client assessments
    • Ensure timely documentation in the Electronic Health Record

    Qualifications:

    • Bachelor's or higher degree in behavioral health or related field
    • Interpersonal skills for positive relationships
    • Effective communication skills and proficiency in Microsoft Office
    • Ability to work independently and manage multiple projects
    • CPR/First Aid certification required
    • Valid Arizona driver's license and Fingerprint Clearance card
    • Must pass a TB Test
