Essential Functions

  • Responsible for planning activities which meet specific resident needs that include large
groups, small groups and individual attention.


  • Observe and report resident's attendance, participation, and behavior changes by
documenting and charting.


  • Must be knowledgeable of individual care plans and support the care planning process
by providing supervisors with specific information and observations of the resident's

needs, preferences and report any behavioral changes.


  • Coordinates activity programs with volunteers from the surrounding community by
providing guidance and assigning tasks.


  • Meet and greet all new residents upon arrival to the facility.
  • Assure that established infection control and prevention practices and standard
precautions are maintained at all times.


  • Follow established safety precautions when preforming tasks and using equipment and
supplies.


  • Maintains the comfort, privacy and dignity of residents and interacts with them in a
manner that displays warmth, respect and promotes a caring environment.


  • Communicates and interacts effectively and tactfully with residents, visitors, families,
peers and supervisors.


  • Reports all hazardous conditions, damaged equipment, accidents/incidents and supply
issues to the supervisor.

peers and supervisors.


  • Ensure each Guest receives person centered care.
  • Answers and respond to call lights promptly and courteously when working in resident
care areas


  • Reports all resident concerns to the appropriate department head.
  • Attend and participate in departmental meetings and in-services as directed.
  • Recognize your role as part of the Quality Assurance and Performance Improvement
(QAPI) efforts of your organization.


  • Attend trainings to build understanding and capacity to undertake QAPI work.
  • Carry out QAPI roles and responsibilities as assigned.
  • Follow established policies and procedures in support of QAPI efforts.
  • Any noted opportunities for improvement will be shared with leadership in the
organization.


  • Communicate to leadership upon witnessing a positive outcome of a QAPI project or
detecting barriers preventing project success.


  • Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude),
including adjusting performance and practice in accordance with QAPI initiatives and

findings.


  • Participate in compliance trainings as scheduled and adhere to compliance plan.
Work Environment

This job operates in a health care setting. This role requires regular walking to various locations

around the facility while pulling or pushing carts. This position works mostly in the resident

care areas as well as in occasional outside weather conditions. This role may also come into

contact with patients who may have contagious illnesses.

Physical Demands

While performing the duties of this job, the employee is regularly required to talk and hear. This

position is very active and requires standing, walking, bending, kneeling, and stooping on a

regular basis. The employee must be able to lift and move items weighing at least 20 pounds.

Specific vision abilities required by this job include close vision, distance vision, color vision,

peripheral vision, depth perception and ability to adjust focus.

Position Type and Expected Hours of Work

This position is part of a health care facility that is open 24 hours a day, 365 days a

year. Rotating shifts as well as holiday and work during inclement weather will be required.

Travel

No travel is expected for this position.

Required Education and Experience

High school diploma or equivalent.

Preferred Education and Experience

One year experience in an Activity Aide role.
