Looking for full time case manager m-f 40 hours per week to work with a caseload of individuals receiving long term care waiver services.
Responsibilities include coordinating, communication, documentation, develop service plans, calculate budgets, follow policy standards, conduct visits, and maintain current compliance overall in client chart.
Temporary procedures in place for Covid-19Pay:
$ $18.00 per hour

Experience:
Case Management: 2 years (required)
