Mental Health Specialist works along with and assists HMH Behavioral Health staff in a grant-funded position providing support to physicians and patients with mental health diagnoses.+Responsibilities
- Provides clinically competent case management services
- May include phone calls communicating with patients families completing forms letters regarding Social Security disability forms Leave of Absence etc.
- Provide referrals linkage to community-based services case management for patients families.
