A. POSITION SUMMARY

The RW Program Manager must effectively manage CareSouth's Part A and Part C programs and all other special projects under Ryan White Services.

B. STANDARD RESPONSIBILITIES:

1. Knowledge and understanding of Employee Handbook.

2. Carry out the meaning of the CSMD Mission and Vision statements.

3. Practice a culture of safety to reduce or prevent risk of injury, claims, loss, or liability by utilizing the Risk Management and Infection Control Plan.

4. Perform responsibilities with a high-quality standard to yield improved compliance, quality, and patient outcome measures by adhering to the Quality Improvement Plan.

5. Be present and ready for work as scheduled.

6. Attend in-service trainings and community events.

7. Work cohesively with team members.

C. CORE RESPONSIBILITIES:

8. Remain knowledgeable of all RW program protocols and inform, with documentation, staff of all RW protocols and make readily accessible all protocols to RW staff.

9. Ensures compliance with all regulatory bodies to assure program integrity and optimal functionality.

10. Serves as Lead for the entire RW Care Act title grants received by the HIV program.

11. Provide training, supervision, oversite, coordination of duties of all Ryan White program staff.

12. Develops, implements, and enforce operational protocols, policies and procedures relative to the Ryan White Program.

13. Effectively manages programmatic services by ensuring all aspects of patient care are provided either internally or externally.

14. Works with administration to obtain and manage formal collaborative agreements with external service providers and collaborating programs throughout the service area to bridge gaps in services.

15. Completes assessments of patients according to department policies & procedures and documents appropriately.

16. Continuously evaluates the needs of patients according to data and trends and implements programmatic improvement initiatives based on this information.

17. Ensures comprehensive treatment plans are developed and implemented for all patients.

18. Ensure proper consultation to patients newly diagnosed and assists with facilitating

referrals to other supportive services.

19. Recognizes emergency situations and responds appropriately.

20. Responsible for grant proposals, organizing and collecting necessary data.

21. Develops, administers, and maintains a system for managing grants from agencies including but not limited to HRSA, TGA-Title I and Minority Aids Initiative.

22. Collaborates with administrative and information technology personnel to implement and monitor grant performance.

23. Assures completion of annual evaluations of Ryan White program staff, documents in employee records to include performance evaluations and personnel transactions.

24. Maintains a quality assurance program in coordination with CareSouth's Director of Quality and in compliance with the CSMD policies, quality program and HRSA/HIV Quality Management program.

25. Communicates with HRSA Project Officer as appropriate and works collaboratively with CSMD administration and leadership.

26. Represents CSMD HIV program in local, state and national meetings; statewide committees; task forces and other groups as necessary and appropriate.

27. Assessments of prior clinic and grant performance, analyzing and identifying shortcomings, setting priorities, designating interventions and monitoring the operational impact of interventions with clinic and agency leadership.

28. Monitoring indicators of clinical quality, , volume, patient complaints, and evaluation of patient satisfaction with the RW Program staff and Medical Provider.

29. Monitors data entered into CAREWare system and manages function data received and to be reported to various agencies.

31. Communicates with the BR-TGA office for all needs of grants, reporting, and quality management.

32. Responsible for the management and timely submission of the RSR (client level data reporting) and all RW non-competing grant reports.

33. Responsible for attending and preparing report for all CSMD huddles, departmental and leadership meetings in collaboration with Director of RW Clinical Services.

34. Performs any duties or delegation as set forth by immediate supervisor, CMO, COO, or CEO.

35. Ensures that all equipment and supplies warranted for adequate functioning of department are available and in adequate supply.

36. Ensures RW Program staff provides patient and/or family significant teaching/ education.

37. .Ability to educate and relay information to patients in a clear and concise manner.

38. Target specific groups served by the clinic - the disadvantaged, the elderly, minority groups, pregnant women, etc. - and design needs-based programs.

39. Organizes and participates in community activities, such as health fairs, school projects, etc. including National HIV Testing Day and World AIDS Day activities.

40. Utilize appropriate resources to accommodate age, education, cultural and communication barriers.

41. Initiates multi-disciplinary referrals as needed and does appropriate documentation and follow-up.

42. Assures informed consent is obtained.

43. Respects and ensures the privacy of all patients.

44. Responds and recognizes emergency situations and responds appropriately to all emergency codes and drills and completes appropriate paperwork.

45. Follows Standard precautions by utilizing appropriate Personal Protective Equipment.

46. Demonstrates knowledge of P&P for the Environment of Care Plans including but not

limited to: (1) Safety and Security; (2) Hazardous Material and Waste; (3) Fire Safety; (4) Medical Equipment; (5) Utilities; (6) Other Physical Environment; and (7) Emergency Management.

47. Maintains open communication with all team members throughout CareSouth.

48. Performs other duties as assigned.

D. LEADERSHIP

1. Continually inform and promote goals and status towards productivity, quality, compliance, and safety/risk management goals.

2. Effectively onboard and train staff for success utilizing onboarding template.

3. Timely complete and review performance with each team member.

4. Motivate and build rapport with team members.

5. Delegate and develop staff for flexibility on the team.

E. REQUIREMENTS

1. Bachelor's degree in health care, social work or related field.

2. Registered Nurse; preferred.

3. Minimum of three years of work experience with PLWHA.

4. Minimum of five years of supervisory experience, with at least two years of experience in this field.

5. Current CPR certification is required.

6. Experience in collaboration with interdisciplinary healthcare teams.

7. Experience in ambulatory care setting preferred.
