Home Health Agency in Washington, DC, is seeking a Health Information Clerk to join our team Must be a Certified Medical Assistant in DC.

Our Home Health Agency is an in-home healthcare provider offering skilled nursing, rehabilitation and non-skilled services for the residents in the community that require support with ADL's that may have a physical and/or developmental disability.

Our staff of RN's, LPN's, MA's, OT's, PT's, SLP's, and HHA's support our patients in their day to day, while improving their quality of life


Our Health Information Clerk is a Certified Medical Assistant responsible for tracking and monitoring all Medicare, EPD Waiver, and Medicaid plan of care admissions, re-certifications, and discharges. *Must be tech savvy
  • The Health Information Clerk also collaborates with the Administrator, Clinical Director and billing department to ensure that processes are in place to facilitate timely development of the clinical documents required for MD approval and signatures. The Health Information Clerk coordinates, and facilitates scheduling of professional staff services (RN, LPN, PT, OT, ST). The position requires the ability to work effectively in a cross-functional environment.

Qualifications:


  • Medical Assistant Certification in DC
  • Must have prior Home Health experience
  • Must be computer literate, and have familiarity with Home health platforms, with the ability to create and revise documents and communications
  • The ability to maintain confidentiality in all matters
  • Excellent organizational skills
  • The ability to perform all responsibilities in a highly independent and professional manner consistent with the organizational goals
  • The ability to work as a team member handling issues in a fastpaced environment.
  • The ability to conduct all verbal and written communication with courtesy, responsiveness, thoughtfulness and care

Responsibilities:


  • Assists in maintaining a high quality of service delivery within the Agency. Assists in maintaining compliance with federal and state laws/regulations, and company's policies/procedures.
  • Responsible for members gaining access to needed services through coordination and integration of medical and long term care services for the purpose of orientation, care plan development, assessment, and care coordination.
  • Followup with MD offices weekly for all pending plan or care and physician order authorization.
  • Collaborates with Intake Coordinator and Case Management Coordinator to facilitate admissions and discharge paperwork requirements.
  • Track and assign all recertifications, and start of cares, discharges and other professional staff tasks in accordance with federal and state regulations
  • Review incoming assessment from DC Care Connect, and coordinate with the assigned nurse for update and/or clarification
  • Develop charts for new clients, files admission, plan of care, physician orders, and other paperwork as needed
  • Develop plan of care discharge and rescind letters in accordance with regulations
  • Update and maintain the emergency plan of care log book
  • Call, fax, mail and/or visit MD offices to ensure appropriate signatures are received, per DC Home Health Regulations
  • Develop admission packets for the beneficiaries
  • Assist with incoming telephone calls
  • Collaborates with Human Resources to ensure that professional staff personnel/contractor records remain in compliance, and notifies professional staff of missing or expired documents
