The Justice Education Center, Inc. will hire a Project Longevity Social Network/Family Case

Manager, who will provide case management and overall coaching and support to members of the social networks of high-risk individuals, including particular focus on the women, and their children who are partners and/or co-parents of high-risk PRESS clients.

The Social Network/Family Case Manager will serve participants with regular (weekly or semi-weekly) meetings, and will provide retention contact to additional participants.

The Social Network/Family Case Manager will provide warm handoffs and referrals to workforce development, housing, basic needs, education, healthcare, and other providers, and will also provide career and life skills coaching.

Additionally, the Social Network/Family Case Manager will offer violence intervention coaching to equip participants with the skills they need to disrupt violence in their social networks.

Deliverables

• Employ and manage a Social Network/Family Case Manager.
• Provide services to families of 30 PRESS clients (30 households) per year.

Activities

• Provide social Network/family case management and overall coaching and support to

members of the social networks of high-risk individuals, including particular focus on the
women, and their children who are partners and/or co-parents of high-risk PRESS clients.

• Participate in multi-agency collaborative case management meetings.
• Participate in the University of New Haven's research and data-capacity building activities.
• Participate in any other trainings, community meetings or activities as required by the New

Haven Office of Violence Prevention.


Qualifications:

• Bachelor's degree in Criminal Justice or related field preferred
• Previous experience preferred
• Strong communication and interpersonal skills
• Ability to work effectively in a fastpaced environment
• Certification in crisis management is a plus

Pay:
From $46,000.00 per year
