GENERAL DESCRIPTION:


Under the supervision of the Program Manager, the Housing Navigator is responsible for care coordination and referral/linkage services for all clients with housing needs.

The Housing Navagator will assist clients with finding housing options while enrolled in the program and secure housing before they discharge.


ESSENTIAL FUNCTIONS:


  • Supports individual clients needs to secure housing.
  • Collaborate and build relationships with Recovery Residency Managers
  • Provide individualized resources and referrals to community programs.
  • Maintain tracking of clients receiving housing.
  • Uploading housing invoices into Microix System for Rental Payments.
  • Conducts Recovery Residency "walk Throughs" with Recovery residency Partners and perspective partners.
  • Regularly coordinate and communicate with Counselors and case managers regarding clients that may need housing or that are currently being housed.
  • Work with clients deemed in need of significant care coordination and/or referral, linkage and coordination services.
  • Link clients with resources that are relevant to support client recovery.
  • Complete intakes and recommends appropriate level of care for clients in need of SUD treatment.
  • Ensures that Medi-Cal eligible clients have active benefits.
  • Provide information, support and follow up services as needed.
  • Participate in treatment team meetings and contribute towards definition of treatment plans and diagnosis.
  • Develop and maintain a comprehensive and current database of relevant resources for persons served by the program.
  • Ensures client records are maintained according to quality improvement guidelines and other contract requirements.
  • Consults with other community, social, health and welfare agencies.
  • Participate as a team member in administering internal and external rules and program standards in order to support client progress toward identified goals.
  • Perform other duties as assigned by direct and executive management
  • Performs drug testing urinalysis specimen collections and screening as required

EDUCATION & EXPERIENCE / LICENSES & CERTIFICATIONS:

Certified or Registered SUD Counselor with the state of California Required
:


  • At least 2 years' experience in a clinical setting, preferably working with clients with substance use disorders and/or mental illness.
  • Ethics and standards relevant to client/staff interactions in therapeutic, case management, and/or health and human services environments.
  • Bilingual Spanish preferred.
  • Must have CPR certification or be willing to acquire on the job.

Pay:
$ $28.00 per hour

  • 8 hour shift
