- Engages with the patient and assists with identifying resources to enhance treatment; works collaboratively with Behavioral Health Director to assist with obtaining necessary outside support for patient.
- Reviewing records and applications; conducting orientations
- Determines patients requirements by completing intake interviews; determining need for medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.
- Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation.
- Monitors cases by verifying patients attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
- Maintains patient records by reviewing case notes, logging events and progress.
- Communicates patients progress by conducting interdisciplinary meetings and evaluations; disseminating results and obstacles to team; identifying treatment influences.
- Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
- Assist in day to day needs for Drug Court such as but not limited to scheduling group attendance, keeping documentation for patient attendance and drug screen results, and coordinating appointments.
- Bachelor's degree from an accredited college or university in social work, social services, human services, or a related field.
- Two years of professional experience providing case management to children and families preferred in a rural environment.
- Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred.
- Knowledge in ECW Health Records EMR System
