Supports quality improvement, clinical programs, and patient care initiatives through chart abstraction, data collection, and other administrative tasks
Responsibilities:
- Compare medical chart notes with HEDIS/PQA/IHA measure specifications and guidelines to determine if documentation meets requirements for quality measures
- Abstract clinical information from paper/electronic medical records and input data accurately into a quality platform to improve measure performance
- Support medication reconciliation postdischarge efforts by sending faxes and calling provider offices/facilities for information
- Pull and prep weekly appointment Gaps in Care reports for the Virtual Pharmacy team
- Obtain medication lists from provider offices for pharmacist review
- Assist with scheduling pharmacist consultations
- Pull Likelihood of Hospitalization (LOH) reports and share them with provider offices
- Directly outreach to patients to assist in closing gaps in care
Skills and Experience Required:
- Bachelor's degree or equivalent experience
- 3+ years of experience in healthcare industry
- 1+ years of clinical chart review/data abstraction experience preferred
- Basic electronic medical record knowledge and ability to quickly learn new software
- Ability to organize tasks and complete projects in a timely manner
- Knowledge of quality measures (ie, HEDIS)
- Excellent interpersonal, verbal and written communication skills
- Attention to detail and ability to follow precise instructions
Additional Information:
This position can be 100% remote
Salary Range:
$20 - $23/hr
