"We are a young, dynamic, multidisciplinary team (doctors, nurses, social workers) providing care and support for severely ill patients and their families in the outskirts of Munich and the districts of Ebersberg. We are currently seeking a colleague in the field of nursing for our specialized outpatient palliative care (SAPV) services.

As part of this role, you will be responsible for:
* advising, treating, and supporting severely ill and dying patients and their families in their homes * providing guidance, instruction, and support to family members and caregivers * collaborating with other healthcare providers involved in the process, such as general practitioners, pharmacies, network partners, and other care providers * coordinating the treatment process with the medical and social work colleagues involved in the care process * maintaining open communication and teamwork with all involved parties."
