This is a Full-time Case management Position Monday through Friday from 9:00 to 5:30.

We are looking for a Registered Nurse(case manager) and Licensed Vocational Nurse( Care coordinator)

Job Functions / Responsibilities

  • Plans, Organizes and Directs home care services
  • Completes an initial assessment of patients to determine home care needs
  • Regularly reevaluates the patient's nursing needs.
  • Initiates a plan of care and makes necessary revisions as patient status changes
  • Counsels the patient and family in meeting nursing and related needs
  • Provides health care instructions to the patient as appropriate per assessment and plan of care
  • Communicates with the physician regarding the patient's needs; obtains/receives physician's orders as required

Pay:
$ $53.00 per hour

Expected hours: 40 per week
