Position Title:
Resident Services Coordinator

Compensation:

Position Objective:


To provide case management supportive services in permanent, supportive housing to special needs tenants with multiple conditions and disabilities with a history of extended periods of homelessness.

Services are provided using a Harm Reduction philosophy in a Housing First model which places housing stability as a primary goal.


Qualifications:


  • Bachelor's Degree in social services or related field preferred. 2 years of experience in case management are preferable.
  • Knowledge and experience in addressing the issues of homelessness, mental illness, substance abuse, undereducation, and underemployment.
  • Professional verbal, written, problemsolving, and interpersonal skills to be able to effectively communicate with persons from diverse backgrounds, whether in formal or informal working relationships.
  • A valid California Driver's license and reliable transportation.
  • Ability to be flexible, work as part of a multidisciplinary team, to adjust schedule to changing work requirements.
  • Ability to facilitate group interactions and to motivate clients to set and achieve personal goals

General Overview of Primary Responsibilities:


  • Conduct individual client sessions with residents. Coordinate sessions provided via staff, student interns, and volunteers.
Sessions are held according to the client's needs, up to and including daily contact.

  • Develop and implement the facilitation of multiple group interactions per week at the project thru a variety of staff and programs. Examples are weekly social events, walking clubs, educational classes on a variety of health/selfimprovement topics, guest speakers, support groups, tenant councils, etc.
  • Work with program staff and partners to assist residents to develop, set, and achieve personal objectives as related to HUD's primary focus relating to housing stability, skills and income, and selfdetermination. Services include regular assessment of participant service needs, progress review, information and referral services, and coordination of care.
  • Ensure that all documentation are maintained according to agency policy and in compliance with Continuum of Care and other funding source requirements. Record all interactions with residents that related to their Service Plan Goals, housing status, and all major life events/changes.
  • Ensure the proper recording of services delivered for purpose of Annual Progress Reporting to funding sources and internal program evaluation.
  • Assist residents in issues that arise with property management/ other providers, training on lease compliance, and help them to advocate on their own behalf.
  • Work in conjunction with service partners and assist in coordinating care through multiple service agencies to ensure continuity of service.
  • Assist in building a sense of "community" and "pride" on the property by reducing resident's tendency to isolate and encourage networking among residents/use of natural supports and selfadvocacy.
  • Attend monthly/bimonthly agency meetings to reports progress or issues/challenges on your property.
  • Be on the property to handle emergency situations if they arise. Situations can include individual emergencies/crises and situations that involve the community.
  • Other tasks as assigned by the Associate Director or Executive Director.

Pay:
$ $26.00 per hour

Expected hours: 40 per week
