We are currently seeking a skilled and compassionate Hospice Medical Social Worker to join our team.


Responsibilities:


  • Conduct comprehensive psychosocial assessments of patients and their families to identify their specific needs and concerns.
  • Provide counseling, emotional support, and crisis intervention to patients and families coping with endoflife issues.
  • Collaborate with interdisciplinary teams to develop and implement effective care plans that address the psychosocial needs of patients and families.
  • Coordinate community resources and referrals to ensure access to appropriate support services, including financial assistance, bereavement counseling, and other necessary resources.
  • Advocate for the rights and preferences of patients and families, ensuring their voices are heard and respected within the care team and the community.
  • Maintain accurate and timely documentation of all patient interactions and interventions in compliance with organizational standards and regulatory requirements.
  • Participate in ongoing education and training to stay updated on best practices in hospice care and medical social work.

Qualifications:


  • Master's degree in Social Work from an accredited institution.
  • Current state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent certification.
  • Minimum of two years of experience in medical social work, preferably in a hospice or palliative care setting.
  • Strong understanding of the physical, emotional, and spiritual challenges faced by patients and families at end of life.
  • Excellent communication, interpersonal, and counseling skills.
  • Ability to work collaboratively in an interdisciplinary team and adapt to changing environments.
  • Empathy, compassion, and a commitment to providing holistic care to patients and their families.
