Essential Functions


As part of the Community Housing Department, this position develops, implements, and manages Ignite?s individual and shared community apartments for Next Generation (Next Gen) Housing, Clustered and Scattered Sites Apartments (CaSSA) and 3rd Generation (3G).

The Community Housing Case Manager will provide client services as well as implement program policies and procedures in accordance with Ignite?s values and mission.

The Community Housing Case Manager will ensure that all programs operate in full compliance with funding/donor agreements, the position also serves as the primary case manager for Next Gen Housing, CaSSA, and 3G clients.

The Community Housing Case Manager will identify and secure housing resources in target program communities and assist youth to succeed in independent housing.

The Community Housing Case Manager will ensure that service models, client deliverables and programmatic outcomes are aligned with the agency vision, mission, and values, and adhere to the core tenets of harm reduction, trauma informed care and positive youth development.

This position is responsible for ensuring outcomes and deliverables are appropriately measured and reported and demonstrates an understanding of and commitment to the mission and core values of Ignite.

This position reports to the Senior Director of Community Housing and participates in on-call rotation.


Duties
The following duties are the essential functions of this position. The successful employee will be able to perform these essential functions with or without requested reasonable accommodations.

Program Management

  • Under the supervision of the Senior Director of Community Housing, oversees the overall smooth functioning of Ignite?s Next Gen Housing, CaSSA, and 3G Programs.
  • Tracks program service delivery and outcomes.
  • Maintains documentation regarding clients? participation, cooperation, housing adjustment and maintenance, and other records in compliance with agency and external policies and procedures and applicable licensing standards.
  • Coordinates and facilitates activities, events, and community service events for clients enrolled in Next Gen, CaSSA, and 3G.
  • Participates in oncall rotation.
Housing Transition, Placement, and Case Management

  • Collaborates with Ignite Housing Navigator serves by connecting youth to locate apartments and negotiate leases with landlords.
  • Distributes client transit cards, food cards, or gift cards. ? Assists program participants with moving into apartments, including arranging for transportation of personal property, orientation to building location, facilities, and local community services.
  • Orients program participants to safety guidelines, program expectations, responsibilities, and policies, which must be adhered to while residing in program apartments. Policies include those per Ignite and imposed by the leasing company.
  • Ensures program participants understand their lease and terms.
  • Assists program participants who successfully meet program requirements to assume apartment leases or secure other safe and stable housing.
  • Participates in program team meetings and other meetings as necessary, including staff trainings.
  • Serves as case manager for Next Gen, CaSSA, and 3G clients individually on topics such as budgeting and money management, employment training and placement, community resources, apartment upkeep, and school and work issues, in addition to goals set in participants? Individualized Action Plans (IAP) and Savings Plans
  • Maintains regular progress notes of all client sessions, collateral contacts, referrals, and ensures that all client files are up to date.
  • Assumes other responsibilities as assigned by the Senior Director of Community Housing.
General

  • Maintains documents regarding participant?s cooperation, daily activities, and other records in compliance with agency and external policies, procedures, and applicable licensing standards.
  • Member of committees and teams including, but not limited to:
  • Community Housing Team
  • Client Support Team
  • Participates in and contributes to regularly scheduled supervisory and team meetings.
  • Works with volunteers and supervises volunteer tasks as required.
  • Performs other duties as assigned.
