CareOne at Livingston NJ (Acute Rehab Skilled Nursing Facilities) - Hiring for MDS Coordinator to join our team

Responsibilities:


  • Provide direct patient care, including assessment, planning, implementation, and evaluation of patient care needs
  • Collaborate with healthcare team members to develop and implement individualized care plans
  • Administer medications and treatments as prescribed by the physician
  • Monitor and document patient's progress and respond to any changes in condition
  • Educate patients and their families on healthcare procedures, medications, and selfcare techniques
  • Utilize electronic health record systems such as Athenahealth to maintain accurate and uptodate patient records
  • Participate in utilization review activities to ensure appropriate use of healthcare resources
  • Provide medical management support to patients with chronic conditions
  • Stay updated on current medical guidelines and best practices

Requirements:


  • Valid nursing license in the state of [NJ or Multi-State]
  • Minimum [3] years of experience in acute care or similar setting preferred not required
  • Strong knowledge of Medicare guidelines and regulations
  • Familiarity with utilization review processes and medical management principles
  • Proficiency in electronic health record systems, such as Athenahealth
  • Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
  • Ability to prioritize tasks and manage time effectively in a fastpaced environment
