Job Summary:


Here is an opportunity to join the EldaCare Home Health Services, Inc. The agency provides care to families with non-medical and medical
services in Philadelphia and designated areas of Bucks, Delaware, Chester, and Montgomery counties. In this role, you will provide hands-on nursing care and education/training for patients, families, and caregivers to equip them with effective self-management skills for their illness and conditions. As a Registered Nurse, you will lead and coordinate care for interdisciplinary team members. Responsibilities for the Registered Nurse:


  • Completes records and reports necessary for patient service and administrative requirements as determined by EldaCare.
  • Attains personal professional growth and maintains clinical competence.
  • Establishes and maintains effective working relationships with patients, families, peers, supervisors, and community agencies.
  • Meets all performance standards, including those for quality and quantity, as defined by EldaCare.
  • Identifies the need for services of other health care disciplines or agencies and initiates the appropriate referral.
  • Participates in interdisciplinary team conferences; facilitates coordinated and comprehensive plans and goals for EldaCare patients.
  • Communicates regularly with other health care providers to develop coordinated plans for patient care.
  • Supervises Home Health Aide in completing the established plan for patient care, and evaluates the effectiveness of the service.
  • Participates, as assigned, in implementation of the Agency's Quality Assurance Program.
  • Participates as appropriate in EldaCare educational activities, including new employee orientation and professional student clinical training and staff education programs.
  • Interprets EldaCare services and programs to individuals/families and personnel of other agencies.
  • Participates, as assigned, in community programs.
  • Complies with EldaCare Policies and Procedures.
  • Performs other duties as assigned.

Assessment Duties and Responsibilities

  • Completes a comprehensive assessment, including OASIS items, that accurately reflect the patient's current health status and includes information that may be used to demonstrate the patient's progress toward achievement of desired outcomes.
  • Completes a review of all medications the patient is currently using in order to identify any potential adverse effects and drug reactions, side effects, and noncompliance with drug therapy.
  • Verifies patient's eligibility for the Medicare home health benefit, including homebound status.
  • Fulltime RNs will be required to perform between 3032 visits per week
  • Parttime RNs will be required to perform between 2228 visits per week

  • RNs will be per visits

Evaluation:


  • Evaluates and documents patient/family understanding of home care services.

Knowledge and Experience:


  • A minimum of one year's experience in the provision of direct patient care in acute care or long term care facility.
  • Demonstrated competency in OASIS documentation.
  • Knowledge of home care home base EMR system preferred
  • Experience in home health care preferred.
  • Current license in Pennsylvania as a Registered Nurse.
  • Current Driver's License.
  • Current CPR Certification.
  • Demonstrates good verbal and written communication, and organization skills

$ $36.00 per hour
