STARTING SALARY HIRING RANGE:
$ $28.93
$53,379 - $60,184
CLASS DESCRIPTION:
Performs professional level human services work, which may include supervisory responsibility, under general supervision from an administrative or technical superior.
Work includes human service, administrative, and counseling work; providing counseling and programmatic services to the special populations; and providing assistance in identifying the population at risk, its needs and how to meet those needs.
Organizes and publicizes large events for citizens or special groups.Work also includes providing services needed to carry out human service programs or the operation of a medium sized center with extensive regional responsibility and concurrent programming.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Works and cooperate with coworkers and supervisors at all levels.
- Learns and perform all essential job functions accurately.
- Provides planning services for residents enrolled in federal or State programs that fund community services as an alternative to nursing home placement (i.e., Community First Choice, Community Options, Increased Community Services, and Community Personal Assistance Services).
- Works with program participants and applicants to help them understand their selfdirection options and maximize their choice and control.
- Creates personcentered plans of service (POS), sets goals, and coordinates services based on individual needs and choices.
- Assists clients/families in identifying the providers of the services needed, and monitors care to ensure that services are properly delivered and best meet the client's needs.
- Updates plans of service as needed, recommends changes in services when appropriate, and assesses progress or lack thereof of client's abilities.
- Identifies and connects clients with other community resources as needed to supplement program services.
- Utilizes Maryland's webbased tracking system to document client interactions, enter billing activities, and monitor providers. May require some evenings and weekends.
- Participate in activities related to Emergency Management as requested, including work outside normal business hours
SUPERVISORY RESPONSIBILITIES:
- This job has no supervisory responsibilities
MINIMUM REQUIRED EDUCATION/EXPERIENCE:
Bachelor's Degree or equivalent combination of education and experience.
PREFERRED EDUCATION, KNOWLEDGE, SKILLS, AND ABILITIES:
- Bachelor's degree (B.A.) in social work, human services or related field and related experience and/or training.
- Knowledge/ experience with care management working with older adults and/or adults with disabilities.
- Ability to speak and write English and Korean, Russian, Farsi (Korean or Russian preferred)
