    Visiting Nurse and Hospice for VT & NH covers more than 70 towns delivering superior nursing, rehabilitation, hospice, and personal care services with proven effectiveness, integrity, and compassion. When you join the Visiting Nurse and Hospice for VT & NH, you'll become part of a dedicated team delivering outstanding home health and hospice services that enrich the lives of the people we serve.

    Spiritual Care Counselors at VNH are integral members of a multidisciplinary health care team that provides skilled nursing and rehabilitative care to patients, allowing them to receive the medical care required in the comfort of their own home.

    The Spiritual Care Counselor will:
    • Assures spiritual assessment of patients and families in the hospice program and appropriate services are rendered in a timely manner to meet regulatory guidelines.
    • Provides direct spiritual care to patients and families/caregivers
    • Serves as liaison and support to community chaplains and spiritual counselors
    • Documents direct services and ongoing communication with community resources
    • Attends patient care conference and group meetings as a member of the interdisciplinary group
    • Provides for funeral or memorial services for patients as requested
    Minimum Qualifications:
    • Master of Pastoral Care or graduate of Theological Studies from an accredited university preferred , but ordination by an accredited seminary will be considered.
    • Two years of hospice experience preferred.
    • Experience with database software or electronic medical record software.
