At Abiding Comfort HomeCare, we are seeking a compassionate, dedicated, and experienced Non-Medical Home Care Coordinator to join our team.

As a Care Coordinator, you will provide essential support and assistance to our clients and care professionals.

Your role will be to supervise services and support care delivery to clients according to Abiding Comfort policy and procedures.

Clients are primarily located in the Phoenix Valley (Scottsdale, Paradise Valley, Fountain Hills, Tempe, Mesa, Chandler, and more.)

JOB RESPONSIBILITIES

• Conduct new client Home Assessments and Supervisory Visits as per schedule
• Document any and all communications, issues, and changes in CareTime. Make sure messages are followed up and/or returned promptly
• Communicate all staffing issues with HR staff (changes in schedules, calloffs, client/direct care staff schedules, etc.)
• Maintain effective communication with clients, case managers, direct care staff, and office staff
• Emergency fillins
• Oncall afterhours as needed
• Participate in staff meetings
• Any other duties or responsibilities given by the Owner/Director

KNOWLEDGE, SKILLS AND ABILITIES

• Must have a related degree and/or experience in home care, longterm care, or care management.
• Required to pass a criminal/background check.
• Obtain and maintain a Level 1 Fingerprint Clearance Card.
• Current AZ driver's license.
• Must provide three (3) verifiable references (nonfamily).
• Current CPR and First Aid Certifications.
• Current notification of being free of TB.

Please note that this position may require working evenings, weekends, and holidays based on client and company needs.
