Mental Health Counselor
Position Summary:
The Mental Health Counselor is responsible for assisting the Care Coordination Manager with enrollment assessments for the HELP Behavioral Health Clinic.
The Mental Health Counselor will also serve other functions on the intake intake team, including but not limited to, answering new client inquiries, verifying insurance benefits, and explaining certified Medicaid services.
Lastly, the Mental Health Counselor will be responsible for targeted case management services to ensure that client gain access to resources, programs and other community services.
The Case Manager provides ongoing professional, technical and clinical support through comprehensive assessment, planning and treatment, with a focus on coordination of care and individual client's needs.
Other Duties:
- Assessing clients' mental wellness, needs, preferences and abilities and using these to develop tailored service care plans
- Working with clients, family and friend support networks and health care professionals to put care plans in place
- Listening to clients' concerns and providing resources as required
- Recording clients' progress including charting referrals, community/home visits and other interactions
- Evaluating clients' progress periodically and making adjustments as needed to improve outcomes
- Following up with clients to ensure they are satisfied with services
Requirements:
- Bachelor's Degree preferred in Psychology, Social Work, or Human Services or related field.
- Minimum of 2 years human services experience required.
- Prior experience working with individuals with mental illness preferred.
- Must have access to a reliable wireless internet connection when working remotely.
- Computer literate, proficient in MS Office including Word, Outlook and Excel.
- Strong communication skills in the form of both written and verbal.
- Successful completion of all mandatory trainings must be completed before direct service delivery (IM+CANS training must be completed within 2 weeks of hire and then annually from the date of the initial training).
